It looks to me like someone with a torn rotator cuff. If you injure your shoulder ligaments or tendons the wrong way, you can't raise your arm to shoulder level, particularly if you are holding something that weighs more than a few ounces in the hand.

I have a neurological condition called Essential Tremor with an "action tremor." I drink water exactly like this to try to minimize the hand-shaking that goes with lifting up a bottle or cup. And it is also hard to "aim" for the mouth when your hands shake. If I don't steady the bottle with 2 hands then I usually end up spilling the water on to my chest. p.s. I severely dislike Trump but I don't think he is doing this to be weird.
