You have to make the document by yourself, and you need to get attestations from tehsildar or other local governing authorities. then you have to get them attestation from home department and then central attestation is also needed.

Hello, if you are planning to go for Masters then it is not required but there is a possibility that they ask for one in case of Bachelors.
