Because New York is the most famous, but west of the Mississippi was dominated by the chicago mafia, going all the way to Las Vegas, which the movie casino was based on. But you also had the Cleveland, Detroit, Kansas city mob who were just as powerful as new york 5 families. Also In the south you had carlos Marcello out of new Orleans and in Florida you had the traficante mob family all who were just as powerful if not more powerful than any new Yorks 5 families. Look up these crime groups, new york was just one, and could not really operate outside the east cost without permission from those families I've mentioned.
