    The Mayo Clinic and Framingham statin risk calculators say for people with my numbers, at age 65, there is a 9% risk of a cardiovascular event in the next 10 years. They point out that means 90% of people with a similar profile will NOT have a cardiovascular event. WITH a statin, that risk comes down... to 7%. So for a 2% advantage, why would I go on a statin?
    My primary care doc doesn't know about those risk calculators. All he cares about is that my total cholesterol is 401, LDL is close to 250. He doesn't care that the HDL is 65 and the triglycerides are 72. And the LabCorp NMR Lipoprofile test says of the VLDL: "Test not performed. VLDL levels not sufficient for VLDL size
    determination."
    I have told my doc I'm not interested in a statin, and just make a note that patient refused or is noncompliant. Not much point in changing doctors, most of them think the same way and it is hard to find one who is up to date and familiar with a low carb lifestyle.

      I'm in the same position.was told I need to take statin. I said I wanted to find a natural way to get the right levels.
      There a few low carb doctors in Perth. So I'm going to try one.
      But perhaps the VLDL is just the difference after you subtra t HDL & VDL from the Total Cholesterol...

      I have similar numbers after 10 months of Carnivore . Lost almost 90lbs so far and other numbers are good .
      My concern is my CAC score went from 72 to 312 since 218 . And my small particle ldl numbers are still bad . Not sure if it’s worth me taking a statin considering this . He wants me on 20mg crestor to lower my ldl of 230 . My HDl is up to 55 now and Triglycerides are 73 .

      @@grantharding8868 There’s a cardiologist, Nadir Ali, who was asked about statins in a RUclips channel in the past day or two. He’s a proponent of low carb lifestyle.
      He said even with the presence of some calcification, a statin has little relevance for primary prevention.
      Have you tried the Mayo Clinic statin risk assessment calculator? You can key in your numbers and see where you stand in terms of risk. Try that calculator with ‘no statin’ choice, then with. That will show you the potential benefit of taking one. In my case, a 2% difference wasn’t enough to persuade me.

    One of the problems with most doctors is that the insurance companies tell them they have to put you on a statin if your cholesterol is over 200. The private doctors that have joined “ groups” also want you on drugs so they can get you back in their office every 3 to 6 months for refills. It’s a wallet biopsy! I got that term from you.

      Man, I’m confused! VLDL dumps triglycerides into the fat cell. It then changes into intermediate density lipoprotein when it picks up “the good stuff” from the fat cells. It travels around the body dumping its load. It then becomes small enough to be called.LDL. So if you are healthy, Most of your LDL will still have quite a bit of the good stuff in it, and be considered pattern A. If your LDL is pattern B, then you have too much insulin in your body, and you never made very many intermediate density lipoproteins in the first place.. so you’re intermediate density lipoproteins dumped their load into the fat cells, and then never picked up anything from the fat cells.(Pattern B). Did I get that right?

    According to Malcom Kendrick, lipids found within the walls of arteries come from the breakdown of the lipids contained within the walls of clotted red blood. The clots are in response to damaged endothelium, not to cholesterol transported in the circulation. This is why we do not see atherosclerosis in veins. Veins and arteries carry the same level of LDL, IDL, HDL, triglycerides, and VLDL. It is the high pressure in arteries that augments the damage of endothelium, clot formation, and subsequent atherosclerosis, caused by smoking, autoimmune disease, certain drugs, sickle cell disease, hyperglycemia, and infection, among others. Macrophage foam cells clean up the lipids left over from clotted red cells.

      I think this vid did not necessarily conflict with your description of of Malcom Kendrik’s findings from what I gleaned from this video, in that, healthy artery walls do not “dock” with cholesterol particles and unhealthy ones, such as you describe with “blood clots” provide a docking point and thus further damage.

    We don't have to forget that high glucose levels is just one way to damage your blood vessels. You can have a very healthy lipid panel but still have your blood vessels heavily damaged and inflamed, and full of atherosclerotic plaques. Smoking and omega-6 can do as much damage as glucose, and a healthy lipid panel may not protect you from that.

    Stopped taking the statin after first 30 days of compliance. Let's see in the fall after new blood work. Haven't told my doc. Keep getting refill that I don't take. After that bloodwork in Sept I will let her know that I haven't been compliant. Will let u know. I'll say I researched on Google. Never done this before

    Tired of saying no to my dr for "no statins" when TC is 325....BUT hdl:tri ratio dead on at 1:1..Told her I was going for an NMR lipid panel to understand what kind of cholesterol particles I had before I take a statin. .....quote from dr. was "I'm not sure how I will interpret them"....She only saw the first number which was my actual particle count of 2909 (very high on chart)......and never looked at all my great large fluffy sizes. The sdLDL's were "unmeasureable" because i had so few...., all on the "low risk for CVD, and HOMA-IR wasn't even on scale because so low for insulin sensitivity. She still wants me on a statin, I said let me be your N=1 experiment for no statins!....lol

      We need to be our own advocate and good on you for sticking to not taking a statin. Even with a CTA calcium score of 0 at 66 years old and being metabolically healthy after changing to low carb lifestyle in Oct 2021, my cardio recommended low dose statin to which I said no thanks. At my next appointment with GP to get follow up blood test results I thought she was going to have kittens and she told me that I was at a high risk of heart disease. My LDL had gone up even more (from 250 to 300) and she was focusing on TC/HDL ratio and I said I look at the trig/HDL ratio, which was good, and my fasting insulin had gone from 7 down to 5. When I requested an NMR lipid panel from the GP she told me that she would research and discuss at the next appointment. The next appointment she said that because she was not able to interpret the results that she was not allowed to request it. A different doctor, who is interested in low carb, did give me a referral for it. I then did some more checking online and found Dr Paul Mason's explanation of cholesterol where he shows that you can predict the size by the other cholesterol numbers. I fell in the large area (even though I have high LDL). I had to pay out of pocket for the test and decided not to get it and felt that it was better to spend the money on good quality food than on a test that may have been unnecessary.

      @@jobrown8146 I paid out of pocket too for the NMR test, but went through "ownyourlabs" and paid $61. That sight was suggested by Dr Phil Ovadia, (Stay Off My Operating Table) and was well worth it. Can request most any type of blood test.

    Low carb diet, total cholesterol 280, triglycerides 50, HDL 70, vLDL 6. Stopped SAD diet 3 years ago and switched to low carb, whole foods animal based diet. No sugars, grains, flour or any prossessed foods. Feeling great. Funny thing is spent most of my life with 180 total cholesterol and got sick eating oatmeal and low fat crap. I switched to animal foods based diet and now am way healthier with 280 cholesterol.

    Thank you Dr. Cyves! I believe that I’m much better informed after this. After following a carnivore diet for 4 years now I was recently told that I should consider a statin. I told the Dr that after the research papers, books and courses that I have read/done ( Noakes Foundation) I’d prefer not to. My trIigs, hdl, total cholesterol were really good. ldl was elevated …Doc was concerned….I was not. No number for VLDL was given.

    Thank you, Doctor Cywes, for this lecture. I am a very active 70 year old and working on getting my protein intake to appropriate levels--it's a journey! I just got my latest labs. Total Cholesterol: 441; Trig: 55; VLDL: 3 (low?); HDL: 129 (high?); LDL: 309. My cac score was 82 and is primarily in the descending aorta (?); My liver enzymes have been really high but have now dropped into normal/almost normal range (I believe my liver and cac score are due to my addiction abuse from alcohol, sugar, grains, cigarettes for many years--NOW free free free from substance abuse!!!). Any feedback is much appreciated. Don't quite understand how to reach you for a consult but would be very interested. I will join your Patreon today!

    Based on this discussion, it seems to me the question is "what is causing the damage to the blood vessels?"

      Sugar! It Thickens the blood which itself can damage the arterial walls, especially at branch points. I used to think it couldn’t be the blood or it would also plaque up the veins, but they are slacker and not under such pressure, and the branches are converging not dividing as the blood returns to the heart, so the blood isnt damaging the walls. Sounds daft but those blood cells are hitting at the split of a Y shape as the arteries divide and become narrower.

      I highly recommend reading Dr. Malcolm Kendrick's book The Clot Thickens. Just remember these are all theories, as the Diet-Heart hypothesis is still a theory because after 60 years they still can't prove it. They just reinvented it several times after one version gets debunked. But Dr. Kendrick has devoted his life to the question of what causes heart disease and his theory makes a lot more sense than all the various incarnations of the Diet-Heart hypothesis, which demonizes LDL without any explanation of the mechanism for exactly how it's the problem.

    Thank you for all the info. I have a NAFL which is like 2 times the size of normal, have been prediabetic and very obese for years and years. I have been carnivore for less than 8 months and seen positive shifts in my numbers, as you describe. It seems to be taking a long time to do so and I have a long way to go yet. It would be interesting if you were to describe the process in terms of lipids that those of us who are trying to get healthy again are going through. I think it would be a great video. I know I would watch it.

    Doc Shawn Baker recently got some sort of Cholesterol monitor, similar to a CGM. He returned on a long drive from an event, fasted, did a workout, was surprised how much higher his cholesterol particle count went for a while. Broke his fAst with his big steak dinner. He said the monitor is perhaps 10% off. But nonetheless, it proved how dynamic particle counts can be during the course of a day. That was new information tome and proves the lipid energy model.     Bone on bone knee pain. Getting infections of gell 2x per year. After Lipitor 2 mos. ago hurt from arms and legs. Major arm hand and leg weakness. Fell from leg weakness. Couldn't open bottles.

      Statin side effects are no joke. They do serious muscle and brain cell damage, because cholesterol is in every cell and is a big part of our brains. If you could snap your fingers and have all the cholesterol removed from your body, you would immediately drop dead. This the damage to human health that comes from demonizing cholesterol, saturated fats and LDL. Yet 60 years later they still can't prove the Diet-Heart hypothesis. They just reinvent it when one version gets debunked. It's madness.

    Ok, started carnivore/KETO 6 months ago, lost over 100lbs, cholesterol went from 180 to 270, dr wants me on statin now. Went to cardiologist to get checked out. Kinda flips out that eat meat/fish/poultry everyday now, and very little vegetables. Sets up tests, did an echo cardio gram- all good, sets up an ultra sound of both carotid artery’s- absolutely zero plaque/blockage. Now he wants me to get a radiation injected cat scan of the heart to see if he can find anything in the heart arteries. I said not crazy about being injected with radiation and another drug to lower heart rate under 45 for the cat scan. I asked about CAC scan, he said not accurate nor detail enough. I feel like he trying to find a reason to find something to say “yes” we can cut this guy open and shove stints in him. 48 years old, not chest or pain complaints. Blood pressure 108-118 over 68-76 average.

      Im 46 and i had to argue with doctor for 30 minutes before he authorized cac test - which he said is a good test but we should just look at total! cholesterol. There are tests that show the specific kinds of plaque buildup, but it seems the cac is a good start.

    Small change needed to this video for ideal total cholesterol, first mention you said 200-500 then later 300-500. I felt probably the first time 200-500 is what you wanted to say the second time. Great video, and, for Cardiologists not to know and/or continue to study in detail their chosen vocation is beyond belief. My Cardiologist when I asked if I should have a CIMT did not have an idea what that test is, for example. Also, when I experienced a PSVT for the first time he prescribed high statin levels, who knows why as PSVT’s are caused by an extra electrical connection in the heart. Last I checked Statins do not act as a unique form of Ablation.

    My doc tried to put me on a Statin because she discovered I was Type 2 diabetic. I investigated this drug and refused to take it. I told her no thanks and we got into a disagreement so I stopped going to this pill pusher.I am in my late 70's and doing fine.

    What's interesting is my trigs (50) are optimal and have been for over a year, yet I can't optimize my BG levels on strict Carnivore. The best I can do is 90's to low 100's. I produce enough insulin based on fasting insulin (7) and c-peptide (1.4 - 2.2). The only thing I can do as a strict Carnivore is reduce caloric intake, specifically protein to keep BG's at 90's. Been that way since I started Carnivore Sept 2022.

      Sounds similar to the issue I have: HDL 90, trigs 60, fasting insulin>10, glucose stable at 90 (while fed, 70 while fasting) on carnivore. This started shortly after my COVID infection about a year ago. Insulin being too high for my physiology, I really struggle with weight. Drs are clueless

    I'm currently on ZERO statins or any other prescribed drugs, and every day I fight to keep that way because I cannot afford to go to the doctor even with my insurance. Why? Because I have to spend $1500 out of pocket before they BEGIN to pick up anything at 80% until I hit my annual max. My insurance really does suck.

      The general rule in the low carb space is about 1 gram of protein (per the nutrition label not the weight) per pound of ideal body weight.

      No VLDL? I had that on my test last week. The LabCorp report said there weren’t enough to measure, although it gave a size for the VLDL particle. Need to learn what that means.
      Total cholesterol 401, LDL 230, HDL 65, triglycerides 72. Insulin level below 25.

    VLDL was < 1 which Labcorp FLAGGED, lol.
    Triglycerides 46.
    I took myself off statins and went keto-carnivore when I saw a fasting blood sugar of 107 and a drop in HDL.
    My Dr ignored EVERY POSITIVE MARKER ON MY RECENT BLOOD TEST and tried to double my statins due to LDL going up. I have bottle(s) of Statin to give him next visit.
    I’m ready to go to the mat with him.

    8:55 wrong again, the liver typically stores ~100g of glycogen, not 400-600g

      Typically. Yes. But in NASH and on high refined carbs liver can store 4 to 600 g as measured from biopsies and calculated in glycoselated livers. 100 g estimation is mostly in athletes bc data comes as a comparator to muscle glycogen. Then need to look at g glucose per g 1:4 glycogen

    Been on a 20mg statin for 3 years. Havnt had blood work done since then ( BC Canada). Would love to go off the statin.
    Thinking about how to go about that.
    If I get blood work now it will be under the influence of the statin.
    Maybe test now, then reduce dosage and ultimately stop ( over what time period?) , then test again and compare to Dr Cywes’s recommendations.
    I don’t expect any direction or encouragement from my Canadian medical advisers .
    Any suggestions or advise on how to transition off statins would be much appreciated.
    Btw, Dirty carnivore life style.

    I understand that statins do more harm than good, at least for people who have not had a heart attack. But I am unclear about statin use for after a heart attack, like my husband. They put him on Lipitor. I am most fearful of it causing dementia.

    I'm just starting to understand this and plan to rewatch. Had CT scan w dye, 50% mix in the aorta (think that's what is named, the one that pushes blood from one side of heart to other) only in that area. Squeaky clean everywhere else. The "widowmaker" heart attack spot. Doc mentioned a statin before, but I said no. I'm HCM w Rasopathy gene making it worse. If that 50% is calcified, will I need something? For short term? A statin? I just began carnivore 5 days ago.

    I just had a heart attack 3/07/24 after a triple bypass 7 months ago. I have been strict carnivore for over a year. The doctor is waiting me to take statins, and my wife is really wanting me to quit carnivore and take all of medicine prescribed. I'm scared and don't know what to do

      Excellent question... yes, I'd like to know this too. And especially if someone has already had a heart attack back in May & now has 3 stents in 2 arteries blocked 90% & 100%. My husband's CAC score is 5,000 but that's to be expected due to how he has confirmed blockages. All his life he's been high carb diet, he's super lean (11% body fat) never had elevated cholesterol levels, reasonably active, non diabetic, non smoker, a glass of wine once or twice a week or an occasional beer. Has always eaten whole foods, never any junk in the house but very high carb, like 🍌 & old fashioned oats for breakfast etc. Hardly any red meat, nor sugary cereals etc... Mostly lean meats like turkey or chicken, heavy grains, beans, peas, fruits etc mostly he's eaten the high carb Mediterranean Diet. Imagine that!!!
      Now, since May, I've had him doing Whole Foods high fat, protein focused Keto. The exact opposite of what they recommend for heart attack patients. We're under the general standard cardiac care here in RI/MA areas because we'd like to have someone close by where we live to look to in case we need something but we've also decided to pay out of pocket & be under Dr. Ovadia's care so him & his Team have been so helpful & informative. Only "problem" according to his cardiologist & lipidologist here locally is his almost 400 LDL now. They're saying he NEEDS to be on a statin ASAP due to his incredibly high LDL & since he's already had a heart attack but they ignore everything else. And they ignore the fact he still had a heart attack despite eating the Mediterranean Diet & despite his cholesterol never being elevated etc.
      Dr. Ovadia has looked at his lipid panels & says he most definitely qualifies as a LMHR & that's why he responding to Keto with elevated LDL for the first time in his life. His particle sizes & shape are phenomenal & Dr. Ovadia says he wouldn't have him on a statin. But you know how it is. One is always seeking for confirmation. We DO NOT want him on a statin but he's already had a heart attack! Mind you, he's never had high cholesterol & still had an event. Now that he's on Keto, it's when his LDL shoots extremely high but it's all the fluffy ones & the small dense ones are so low that recently, in a consultation with Dr. Ovadia, he explained they're immeasurable. So that's great news! But it's a battle because he's already had a heart attack & has super high LDL now on Keto.
      Anyway, one is always looking for confirmation, you know! So just wondering what Dr. Cywes' take is on taking statins in a situation less than perfect... you know!

    What if I am a Type 2 diabetic 4 years keto and I have a rising A1C of about 7.0 with no spikes but a very high fasting blood sugar of often 160-180, but I have got my triglycerides down to 75 and my HDL up to 50? I can't measure VLDL in Taiwan, so I don't know about that. My fasting insulin and C-peptide were normal the last time I checked, and I am not obese and I do exercise. Is my very high A1C and very high fasting blood sugar driving my high CAC of 265 score even higher? Do I need to eat more fat and less protein? Can anyone please advise? BTW, I think I will also try Dr. Cywes online consultation service. Thank you.

    My hubby had all good numbers with his heart meds and eating low carb 80% of the time. But then he cheated at work and was eating donuts etc and tripled his cholesterol numbers. So i have him on strict low carb again but dr of course have him prescription for statin but then i remembered his heart dr after a quadruple bypass took him off them as he felt sick and couldn't function. Im hoping he can turn his numbers around again. PS i took semiglutide once for couple of months, got lump in throat, scared the crap out of me and got off it.
