You missed a few VERY hidden appearances in some pretty bizzare places. In black Mesa east his face can be seen in one of the powering machines, placed on a texture. It was found only a few years back.
I remember reading on Overwiki that apparently Weatley was originally going to mention the G-man indirectly when Chell wakes up at the start of Portal 2.

In HL:A, in the ending sequence you can see it in the scaffolding to the left of the Helicopter too
you left out his appearance during the Gorden part of Alyx's epilogue when he's on the catwalk to your left
