During the operation, the target breaks through a wall into a neighboring apartment and takes the resident hostage. Hondo and Street throw a flashbang through the gap before storming in. Initially, looked like the flashbang caused the hostage to hit her head on a table, killing her, which is backed up (of course) by the target.
However, the team eventually finds a surveillance camera hidden in the apartment's smoke alarm (put there by the building's owner), showing that the target grabbed the hostage and threw her down, which was when she hit her head. The flashbang went in just a couple of seconds later.
It turns out she died way before the flash bang was deployed
