I don't necessarily see Pinwheel's lore as disturbing, it honestly just feels more sad to me.

The most disturbing moment in bloodborne is definitely the quest of Gascoigne's daughters

Yeah no matter what she dies. She leaves her house and dies, heads towards Oedon church and dies, or to Iosefkas clinic and gets turns into a creature

Ludwig is probably the most disturbing thing I've ever seen in a video game.

In the long corridor leading towards Velstadt in the Undead Crypt, if you look at the ceiling you will see many upside down statues. It was a little creepy for me. It's also the first time I saw the fire-afraid creatures in No-Man's Wharf. And my first encounter with Darklurker. I really had never seen anything like that in the game until that moment, an angel hiding in the dark. The fact that we have almost no Lore information about him makes him more mysterious and interesting. Darklurker made me feel like I had awakened a truly inscrutable Diablo creature. I also want to mention Elena. When I entered the temple in Shulva, the nuns' song scared me a lot. Especially while watching the Sanctum soldiers with their backs turned in ghost form, that spooky song was still going on in the background. It was a pretty scary moment. Finally, we meet Elena. I think the summons Elena summons represent 3 things:
Pigs: Essentially, these are the pigs in Majula and can actually be considered our first challenge in the game. It reminds us of the anxiety that we cannot go on, that we cannot overcome something. Or you might associate these little piggies with every childhood fear and trauma you can think of.
Skeletons: Skeletons represent our fear of death. You will die forever, over and over again.
Velstadt: I think Velstadt represents our fear of losing ourselves and the dark. Take a look at Velstadt. We want to be strong like him, we try to hold on tightly to our ideals like him and try not to lose our faith. But even Velstadt's soul was taken over by the Darkness. What are we in front of him? What if we eventually lose our "self"? What if we can never be as faithful as Velstadt? I don't know, but I believe there is something like this.

Nah for DS1 it's when you find Solaire with the sunlight maggot on him. 99% of prayers would have no idea how to save him on a blind playthrough and now we gotta put him down

If DS3 isn’t stabbing Anri in the face then this list is invalid.

Imo, Seath's lore and story is more disturbing to me. In his madness to attain immortality from his brethen that he betrayed, Seath ordered his Chancellors and minions to intentionally kidnap virgin ladies for experimentation (prominently from churches hence why lots speculate as to why a chancellor is placed in the cathedral before the Gargoyles fight). He orders this so that he may experiment upon them and impregnate them so that they could birth a dragon for him to further experiment upon and find how to transfer dragon scales upon himself.
When you get to the Duke's Archives (Seath's domain), all the Pisaca that attack you are speculated to be all the maidens he kidnapped and discarded as failed experiments.
Edit: Special shoutout to Ceaseless Discharge too.
Also in DS2 since I have mild arachnophobia you already know that Tseldora was the worst for me.

    (Assuming First Time Playing)
    - Ds1 for me is Manus's House and Gaping Dragon.
Manus's House i think i don't need to explain. The Abyss itself shows how disturbing can be. Reject Abyss, Embrace the Dark.
Honorable mention to the Tomb of the Giants, Centipede Demon and Mass of Flesh (enemy) from London
    - Ds2 for me is Shulva's pyramid and Tseldora (i discovered that i have fear of spiders).
The ridiculous *dead* ambience with random songs that are actually toxic spells and Elana with her unnerving song give me genuine fear.
Honorable Mention to Nashandra and the Dark Florest (mostly the fog part)
Special mentions: Queen Ant and Masoquist Tree
    - Ds3 for me is Oceiros and Father Ariandel.
Father Ariandel Rotten body is something that disturbs me, that voice is creepy (same for oceiros). Also the entire Ashes of Ariandel levels's theme.
Honorable Mention to Aldrich on the Cutscene, the Dog from Sullyvahn and obviously my boy prisoner with baby head on Sullyvah's prision.

@@issi529 The cut content might be a big reason for his inclusion but imo cut content shouldn't be an indicator of anything since it's canonicity is questionable at best.

It’s irrelevant how disturbing Oceiros is because you literally stab your bride or groom in the face.

Honestly I wouldn't include things that are disturbing lore wise. It has to be things every player would know just from playing the game and not from a wiki article
