  • The ketogenic diet, or more specifically therapeutic nutritional ketosis, has the potential to help a range of psychiatric conditions, including Bipolar Disorder, Schizophrenia, Major Depressive Disorder, Anxiety, OCD, PTSD, and more. This makes sense because ketogenic diets have been used for over 100 years to treat epilepsy, and the keto diet is one of the most effective interventions for stopping seizures. Since many psychiatric medications are also used to treat epilepsy, that suggests potentially important shared mechanisms.
    In this video, Dr. Bret Scher speaks with a variety of experts - including Julie Milder, Ph.D., Dieticians Beth Zupec-Kania and Denise Potter, and Chris Palmer, MD - about the connection between mental illnesses and seizure disorders, and what that may mean for finding a safe and effective dietary treatment for psychiatric illness.

    Not only mental health, also brain injury. I was bedridden and sleeping around the clock for over a year from a brain injury and cranial nerve damage caused by a virus and meningitis. No medication or conventional therapy helped me. A true therapeutic ketogenic diet got me on my feet again within a month. It’s been a long, hard road back to health, and I’m certain I wouldn’t be where I am today without the ketogenic way of eating.

      I have a brain injury as well.. but i think mine was from SSRI medications. This gives me so much hope, please tell me how do you feel today and if you recommend something else as well. Did you fast as well?

      @@Eliokd I am well. I still have some neuralgia and other loss of function when I am very tired or pushing myself too hard. Rest will remedy this without the slew of drugs I was prescribed. I still have some vestibular difficulty. I can no longer sight read and I need to sound out written words, so you can imagine how slowly I read. There are other losses and half of my face is lower than the other half. I am 70 to 80% back to where I was before the illness. And it’s ENOUGH. I’m satisfied and enjoying my life. I can care for myself without assistance. I can drive. I take long walks. I can work in my gardens. I started my journey back to health with an ultra strict and clean ketogenic diet, very heavy on the fats. I’m now carnivore. Please eat a lot of eggs and butter and fatty fish. Eliminate seed oils and carbohydrates of every kind. I don’t know where you’re starting, and I don’t know where you’ll end up, but I do know you’ll improve. Don’t give up and all the best to you. Oh, I did OMAD for the first six months of my ketogenic lifestyle, no snacking. Also, make sure your bloodwork shows an optimal level of vitamin d. It’s very important for people who are subject to depression.

    I’m doing a ketogenic plan right now but I’ve had one too many carbs here and there from time to time. After listening to Christopher Palmers book “brain energy”, I took it upon myself to significantly reduce carbohydrates to the point of nutritional ketosis in an attempt to attenuate and / or eliminate my OCD, and other psychiatric symptoms

    Thanks, as always, Dr. Scher and Metabolic Mind. I have been trying to resolve an apparent contradiction in my mind! Maybe you can shed some light:
    Based on the research and Metabolic Mind's teaching, it seems highly probably that metabolic/mitochondrial dysfunction is the main driver of mental illness. Nevertheless, mental health issues are substantially increasing within youth populations. I always thought that it took many years (even decades) for a person's diet/lifestyle to damage his/her metabolic health. After all, look how long it takes someone to develop Type 2 diabetes. Therefore, could something else (instead of metabolic dysfunction) be contributing to the rise of mental illness among youth? Or, is it possible that people are being born with a compromised metabolism due to their parent's metabolic dysfunction? Thanks for reading, and I would absolutely love to get your perspective.

    I'd like to add my experience in treating my anxiety and depression. In my case quitting gluten and cross reactivity foods was a game changer. I'm curious about going into ketosis as to how much more I could improve. I'm currently only eating all meats and fruits, most vegetables and honey. Thanks for the information.

    Hi Bret, this has been most interesting. Well done on bringing these experts together! What we need now are many more rigorous trials involving not just child but also adult epileptics. The thing that seems the most discouraging is that the trials to date seem to indicate that a ketogenic approach gives only around a 40% chance of resolving the seizures. Your experts seem far more optimistic so I'd love to know more about the studies that are currently in the pipeline. If epileptics are told that medication gives them an 80% chance of resolving the seizures but a ketogenic diet only gives a 40% chance, then it's pretty obvious what they'll choose.

      Hi Jen. Thanks for your reply. I think another great question, however, is to say, what happens when you combine medications with nutritional ketosis? Do you have a higher resolution rate? And can you achieve resolution with lower medication doses and fewer side effects? I like the approach where they aren't mutually exclusive. And I see that same approach being very helpful for psychiatry as well. What do you think?

      @@metabolicmind Yes, we definitely need evidence from quality trials that combine medication with nutritional ketosis. If the combination can be shown to raise the success rate to over 90% that would really be something. At the moment patients seem to be presented with an "either or" choice and statistically, medication still trumps nutrition. We need solid evidence that the combination is well worth doing.

      Meds really aren’t that effective in psychiatry. Lots of problems with them. I am personally of the opinion keto for mental health is even more promising than epilepsy, especially with psychosis.

      I have to try keto...none of my Dr's believe in it and Im surprised but I'm going to give it hope. I've tried FOUR medications with none working and I'm tired of uncontrolled seizures. I'm desperate to try anything

      ​@@kimberly6200most doctors have been indoctrinated to trust the " healthcare system", but if you haven't noticed yet, healthcare has become a billion/trillion dollar industry, in conjunction with the food industry which is another billion dollar Industry.
      So of course they don't want to find out how to "cure" patients.
      Also Doctors seem to have this Ego/pride prespective since they have gone to 8+ yrs of school, so they know better, FALSE, and thats why they get offended when the patient is telling them what to consider.
      This is my rule of thumb, if a doctor doesn't recommend "fasting" or a specific "diet" as a treatment, and they won't work with you if you ask for a specific test to be done. Then I would say find another doctor who will be openminded and actually read case studies.

    Wish my cardiologist would pay attention to anything keto. He’s so old school. I do keto anyway and I’m in my 5th year of keto & IF. Got off ALL 17 Rx I was on and yet I hear “oh won’t you at least take a statin or baby aspirin?” I said in reply “why? Am I suddenly statin deficient?”

      @@adamandsteve13 the HDL to triglycerides is coming down - 12/15 blood draw shows 1.9 ratio. In my 5th year of keto & IF and brought all my metabolic markers to normal or “near” normal. A1c has been 5.5 since 6/2020 so I’m working on it consciously as I’m in my late 60’s. My “tude” is grateful and joyful. So I know it had a huge impact on staying the course!
      Again my 2 doctors have no clue but I’ve been under their care just over 10 years so they tell me THEIR opinion and I still do the science regardless if they don’t agree.
      I’m guessing unless I take their prescriptions I won’t achieve any health but my blood test results have proven otherwise. It difficult to be respectful to their egos but I don’t want to alienate them in case I ever have an emergency.

      Find another doctor who isn't old school, the old timers have been indoctrinated to trust the system also having so many years under their belt, they feel like they know better !
      At the end of the day, the best doctor is yourself since you know how your body reacts, find a doctor who will work with you.

    I had a tumor removal surgery on January 26 2024. Currently my recovery has been been going well, but I was prescribed to took Keppra 500 mg x2 in the day, but such medicine caused me anxiety, depression and suicidal thoughts. I asked the doctor to reduce the dosis or change the medication, an he just switched to phenytoin whish I think is the same or worse that Keppra after reading all the warnings, hence I decided to investigate and I have been checking the possibility to use vitamins with Keppra or change to Keto to prevent epilepsy (which I haven't had tbh). Hopefully it will work as medication for it, is dangerous.

    Hi!
    I am epileptic and taking Lamotrigine. Right now trying to stay in Keto diet.
    Can someone with Epilepsy take NAC supplements. I heard it is good for the brain, because of the glutamine and "neurotransmisores".

    Interesting. Would love to see the data on adherence vs medication adherence. Seems like many drop out from keto for weightloss because it takes some effort and many eventually need their comfort foods.

    What do you do to have energy to exercise in ketosis? Should you do carb cycling depends of your activity some days 20 g of carbs a day other days 80g?

      Good question. Once someone is keto adapted, they can usually exercise just fine using fat for fuel and remaining in ketosis. Some who do higher intensity or brief bursts of activity prefer carb cycling. There isn't one answer as it depends on how much you "need" to stay in ketosis and what kind of exercise and intensity you are doing. Robb Wolf has good content on carb cycling for jiu jitsu type activities.

    This is food for thought. I was on keto for 3 months and unfortunately I feel rubbish now physically and mentally. I don't think the keto diet is to blame though; it's the fact I was keeping my calories too low since I wanted to lose weight as fast as possible. I have severe bipolar disorder which is controlled by valproic acid. I would love if I could eventually reduce this medication. I fell off the keto waggon but I won't give up. I will also research the exact keto diet that was designed for epilepsy. I myself was eating 70% fat, 25% protein and 5% carbs. Some days I was only eating around 1000 calories (or less) so I think it just wasn't sustainable.

      That's a great point. When someone is in ketosis, they use fat for energy, so if they restrict calories, they run the risk of not getting enough energy. Many people experience weight loss even without purposefully restricting calories. It may be worth contacting a dietician or clinician familiar with therapeutic ketosis.     Thinking of ketosis for 84kg adult male.
    How to calculate total calories and into the 10% carbs 30% protein and 60% fat for example.

      It's tough to give calorie recommendations without knowing someone's age, if someone is trying to lose weight, and their physical activity level. But one place many people start for ketosis is to set carbs at 20g net per day, protein at 1.2 g/kg, and fill the rest in with fat to get enough energy and not be hungry. That approach takes a little trial and error but frequently works well for many.

      Therapeutic ketosis was originally used for kids with epilepsy and a number of studies demonstrate its effectiveness. There are a number of centers, such as Johns Hopkins, that specialize in treating kids with epilepsy with ketosis. And dieticians like Denise Potter and Beth Zupec-Kania have decades of experience in this field.

      It may very well be considering our ancient ancestors were generally speaking, in a state of ketosis due to the sometimes scarce source of calories which required going for prolonged periods of time in a fasted state , plenty of moving / running which would burn up stored glycogen , often very little access to high carbohydrate foods aside from the occasional honey comb , underground storage organ , (not including berries and other fruits which haven’t been genetically modified ). Now, with that being said some could argue their high protein intake would trigger gluconeogenesis, thus halting their bodies ability to enter into a state of ketosis . Nutritional ketosis seems to be a fairly recent discovery.

      Hi J.C. Thanks for your comment. I would push back here and suggest it depends on what your goals are and what's your desired outcome. If you want to change your body's metabolism and change the fuel supply to your brain, then it is likely that ketosis is required. But if you want to improve your metabolic health and lose weight, then many will see improvements on low carb. What do you think?
