- This is a real-time demonstration illustrating technique and patient interaction involved in the Neurological Examination of the Limbs.
The film was produced by practising clinicians to aid the teaching of clinical examination skills. It starts at the point when the clinician has finished taking the medical history and begins the clinical examination.

Wow, so a friend who is currently studying to be a doctor said he thought he had seen me on a video on RUclips that had over a million views and I thought he was having me on. It turns out he wasn’t, and the “educational video” I took part in 8 years ago has turned out to be a RUclips hit! Some of the comments here are absolutely hilarious 🤣. For context, I was playing rugby for Leicester at the time and was offered a few hundred quid to be an actor in the two videos. Each video took a whole day of filming and numerous takes under red hot lights, hence my rather somber mood. The doctor is an unbelievably nice man, he treated me for a serious neck injury I sustained a few years later and got me back playing rugby, which I am very grateful for. As for the dress sense and hairstyle... I’m afraid I have no defence... absolutely awful! I had no idea what ASMR was until now but I’m glad you all get some sort of joy out of this 🤣🤣

well i dont know about the other guy but i personally get very relaxed when watching these vids which is part of asmr
Goatskin2012 some people experience it while others don't. True ASMR is actually a muscular response you body that causes a tingling sensation generally in the scalp in response to certain triggers. Its like goosebumps(chills) but on your scalp or back of your neck.

Thank you Dr for a great video. Can i ask you / others a question of presentation of foot drop post stroke patient case as a medical student. The patient had a young stroke 34 and presents as a foot drop. Swings leg to side to prevent toes dragging and falling. Is able to bend the knee however keeps hyperflaction and swings. Questions for examinations are regarding testing and on gait presentation. would a pt with a true foot drop be able to perform the task of walking on tip toes and walking on heels ?
I just was not sure if there is a big relevance for the tip toe and heel walking testing. Hoover sign is negative etc. Other exams to see if truley is drop foot is needed. I’m a registered nurse just starting 1st year medical school with not much experience with true drop foot to be honest and I appreciate your thoughts and time. Thank you dr and have a great day!

I wonder whether the physician should have asked the patient to close his eyes while using the pin and cotton swab tests. I think the visual feedback could potentially colour a patient's response.

This guy's never gonna make it as a doctor since he hasn't mastered the word "BRILLIANT!"
The areas he does it in can be particularly painful for the patient. Especially if it's difficult to illicit a response on the first time, and in elderly patients, bruising could occur. Doing it on our thumb (specifically where the thumb is above the tendon,) prevents discomfort for the patient, and doesn't require us see anything, we can simply feel the response.

I would have thought that at 07:51 onwards the guy would have been asked to close his eyes whilst the neuro-tip and cotton wool part was taking place, as someone who was having difficulty in detecting could have just said yes when they saw the touches happen.
I'm guessing he simply forgot to.
Maybe. But a short while after for another part of the exam, the doctor asked him to close his eyes
He didn't need his eyes closed for that part. If he has difficulty detecting the cotton then he would have to say No to the doctor, which indicates that the area he touched with the cotton has possible signs of numbness or loss of sensation which could be linked to a damaged nerve.
At 4 minutes it sounds like he's saying shut up instead of sharp

it could yes. but some people, including myself, can see the pin hit the skin, but may not always be able to notice the sensation from it, depending on how sensetive your skin is. :P
