Set gameplay wise aside I notice enemy with three deathblow counter is those who are considered a powerful beast or a monstrous person. Take example of demon of hatred lore wise he could swipe entire inferior ministry and ashina force at the same time. Meanwhile there's isshin a man who stop him. Isshin lorewise was a monstrous person in a war he could be considered a god of war with almost no chance survive encounter against him. Meanwhile 2 deathblow counter are given to extremely powerful and skilled warrior but not necessarily a monster they could rival those with 3 deathblow counter in skills. But they physically isn't as powerful as those with three. Take example of genichiro whose without a doubt a powerful person but he was nothing compared isshin or owl father who also a legendary shinobi and few could considered as isshin rival but owl is shinobi while isshin is samurai in term of skills and experience they may not far apart a samurai always has a stronger physical compared shinobi. That's why old man dying isshin only has two deathblow where he on condition on par with a normal powerful human. This quite interesting considered isshin who is human (let's set aside the fact he was called from underworld by genichiro) who is a human can be on par physically to demon of hatred and a monk who is basically kinda immortal (hard to describe yao/corrupted monk but she considered wicked powerful) and isshin but a normal human who through all of his hardship manage to stand on the same height if not higher with all those things. I also love how this game manage to make you meet two godly powerful opponents (both gameplay and lorewise), in completely unexpected occasion like you finally arrive at fountain head Palace and you see a gate guarded by a monk who you expect to be a regular enemy only to be surprised by her 3 deathblow counter and for her to beat the crp out of you same with demon of hatred where you taking down some of central force army only to be surprised by a mythical demon. Kinda funny considered how much you have been through before that point(gigantic snake, zombies, creature, etc) only to realise that you haven't meet the true monster of this game
I think 3 death-blows are just for exceptional "immortals" fighters. Hear me out. - The corrupted monk is infested by a centipede. - So is the guardian ape. Who, you might reply, has either only one or two death-blow ... Eeeeexcept he's a beast, not a fighter. And the first time you take a death-blow, you cut his whole head off. -And then- you fight him again for the 2nd phase, and afterwards a 2nd time with two more DB in the cave, with his mate. - Genichiro has 2 deathblows but unlocks a third one thanks to the rejuvenating waters, and even when he looses, he comes back. - Ishin, in the shura ending, has 2 death blow, but I'm not sure he would ever had more : he's very much mortal, after all. "Sword saint" Ishin, on the other hand, was brought back from the underworld, by the sacrifice of an immortal being (genishiro), which is why you use the mortal blade and have the "immortality severed" thing when you cut him off. - As for the demon of hatred, well. How immortal the sculptor actually was is impossible to know, but he was preeeeetty confident that a disease with 100% casualties wasn't going to get rid of him. And the flames of hatred DID turned him into a gigantic demonic beast. It's even said several times through descriptions dialogues with him and the old nun that he wouldn't get to the underworld because of the flames. You could even theorize, given how severely wounded you find the divine dragon, and knowing that Tomoe faced him with the black (wrong) mortal blade, that he lost to her long ago, which is why he is defeated after only one DB. Heck, at the start of the fight he's pretty much dead and resurrects because you cut the dragon-rot ridden old dragons of the tree.
