The sound it makes it's real but the sound don't able to recognize from outside. It's only giving sound on car's radio/sound box. If you inside the car you can hear it but if you're outside then it's not possible to hear.
Only if you have that thing at max volume with windows opened, or if you got a damn subwoofer and managed to connect it to the phone. Maybe with some tinkering, you can even get the car to shake with the subwoofer (since it's going to be insanely loud).
