Great breakdown of the employee evaluation system. The previous company I worked with had a similar evaluation system prior to contract renewals, at end of year, but employees did NOT fill out their own evaluation which I thought was a loss. Keeps people on track and accountable.
I think this is an excellent way to run employee evaluations, and I've run many of them this way. Very solid format! Two ideas that could also be considered: 1) I heard recently from another HR person that they have the employee fill out their own copy of the evaluation, while also obviously having the manager/leader fill out a copy of the evaluation. Then, when they meet, they review together. Obviously the meeting would go longer as there will likely be discussion around the gaps or differences in grading, but an interesting concept nonetheless. 2) An employee asked me once if they could receive the evaluation the day before, so they have time to digest it and think through it before the actual meeting. Do you have any thoughts on those two ideas?

I liked your way of doing it but we are having self assessment form where in all goals set needs to be described and rated and then the same is discussed by manager and how it would have been better and what are the learning out of it. Also what they are going to take up in next year. What are there needs from training point of view.

Dude, why do you say the evaluation form is in the body of the video and then don't add it? And you say to people who ask for it o email your assistant to get it, an email also which you did not provide!
I've seen people "Not Give 10's" but then they have NO feedback or concrete advice about How they can improve. So I'd be careful with the "no 10's" blanket statement. Sounds like bullshit sometimes and can have the opposite effect for the employee.
Same employees with high rating, or same low rating...Totally bias . Mgmt already developed their black list or white list to go by. Nothing to do with individual performance

I agree with you! I've got 3 of 5 for Communication skill but no one told me how to improve it
