Possible she made refills as well , so the buyer can purchase in advance or if she is a well established crafter , her costumers knows how to reach her for a refill . Beside of that , I am sure most people would buy it as one time use item , either as a gift for for personal use and would not overthink about the refills and so forth .

I use two sizes for notebooks: 3mm and 5mm. I believe you bought the 5mm, so that can be used as well, especially if you are using a thicker thread, ribbon or even lace...If you want the 3 mm, here is the link: amzn.to/4elrBWr

They would just replace the notepads without the cover.
