The lyrics were written by John Newton, an English sailor who became converted largely as a result of God saving him and his crew-members out of a violent sea-storm that left them desperately fighting for survival. The Music is a traditional tune called “New Britain”, likely of British origin.
According to Wikipedia, the tune was first assigned to the hymn Amazing Grace in 1885 by American composer William Walker, years after John Newton died in 1807. Both the original lyrics and the traditional tune are in the public domain.
