I am not Italian so this is purely outside perspective, It generally seems to be as Raora said when asking a question or when frustrated about trying to get to bottom of something, or to "get to the point". So for me the gesture is almost a very literal attempt to "grasp the situation". I don't know, my brain looks at stuff in a weird way so I could be entirely wrong.
