Canine-Bicuspid Tranposition and Class II Crowded Malocclusion Treated by Insignia® Custom Bracket System
Abstract
An 18 year-old female sought consultation for her crooked teeth. She was diagnosed with a Class II malocclusion with severe crowding, a canine-bicuspid transposition, and a second bicuspid impaction over her upper right side. The treatment plan was to correct the Class II malocclusion by two IZC screws and extract two deciduous teeth. This case was treated with the Insignia system® (Ormco, Glendora, CA), a reverse-engineered production of customized brackets and archwires. It features high accuracy and efficiency. The digital set-up was then designed following a computerized treatment planning. This case was accomplished in 20 months, with crowding relieved, Class I molar relationship achieved, and transposition corrected. The patient was satisfied with the outcome and was pleased with her charming smile.

Dr. Chang, the final pictures show that both upper first and second molars need extra first order rotation, mesial-out and distal-in. It would’ve given more room for UL3 and the buccal segments to seat into class I. With Insignia this can be made to be part of the prescription, which is an incredible and powerful tool.
