BPD Trigger Avoidance Keeps You Stuck | LOIS CHOI-KAIN

  • Dr. Lois Choi-Kain describes the human dilemma of managing our fears so that we don't lose control and harm ourselves or others. Is it good to try to avoid your triggers for the rest of your life? Is it even possible? What's so great about facing your fears in the first place?
    People with BPD are more easily triggered and overwhelmed by their environment, and more destructive when triggered. How do you go about facing your fears and taking risks when it seems so dangerous, and the consequences so dire?
    Dr. Choi-Kain is a powerhouse in the BPD treatment world. Her short bio:
    "Dr. Choi-Kain is a leader in the Good or General Psychiatric Management (GPM) approach developed by her mentor John Gunderson, M.D. with his long-time collaborator Paul Links, M.D. GPM aims to be a generic form of BPD centered care that is more accessible and just plain good treatment.
    Lois W. Choi-Kain, MEd, MD, is the director of the Gunderson Personality Disorders Institute. The institute provides training and supervision for numerous proven treatments, including mentalization-based treatment (MBT), dialectical behavioral therapy for post-traumatic stress disorder (DBT-PTSD), transference focused psychotherapy (TFP), and general psychiatric management (GPM). She works nationally and internationally to expand teaching efforts on borderline personality disorder and its evidence-based treatments and engages in research to study resources for training clinicians who need direction and patients who need access to informed care. With her mentor, John Gunderson, Dr. Choi-Kain developed a training program for GPM and has been expanding its applications.
    In 2009, Dr. Choi-Kain developed the Gunderson Residence, a specialized residential program for adult women with severe personality disorders. In 2013, she founded the BPD Training Institute, a major center for proliferating awareness of and evidence-based care for severe personality disorders. Dr. Choi-Kain has also developed training clinics in McLean’s Adult Outpatient Services for treatment approaches such as MBT and DBT-PTSD."

      Me too...I had an abusive Asian mom so I've avoided Asian female therapists my whole life. She's making me realized my bias is ridiculous.

    "People need corrective experience ... Skiing involves risk and things happen to people". These are some of the most meaningful and powerful words that I've heard in terms of BPD. Great video! I keep replaying these last few seconds of the video!

    "Therapy is only one mode of healing. It's not the end all be all. It's a tool that's a set of customs that are sanctioned by the scientific community and overseen by boards." Thank you for this. I do believe that therapy can be wonderfully healing -- I have had a great therapist myself in the past -- but I think it's very important to put it in context since most people do not have access and since there really are multiple modes of healing and also because there are so many bad therapists out there (not trying to pick on therapists but the gateway to becoming a therapist is not nearly strict enough and they can do real damage). I think a lack of corrective experiences at an early period of development is hugely implicated in mental illness in general. The problem is, corrective experience is capricious. It shows up whenever it will. But thanks to Dr. Choi-Kain for her rich combination of clarity, strength and humility.

      Yes, I read recently there's about 7% of therapists compared to the need, and you figure how many of them are able or even willing to work with BPD, and then how many of *those* are good at or specialize in BPD then it starts looking like the odds are just not that good practically speaking. Personally I believe a LOT more people have to be come psychologically literate in the community so there's a greater level of awareness and understanding, it is just not at all realistic to rely on the expert in todays mental health climate and without a large movement in that direction I don't know where we'll end up. Content like this offers some hope but I think a lot more people need to become familiar with it. For those who have a therapist they like working with, I hope they will look at this information so they can be better helped. It does seem some therapists are not really open to learning new things.

    The ptsd is spot on, it's difficult and can't explain it but it's like getawayyyyy getawayyy. I can tell you 100% there's nothing to be scared of and am behaving irrationally but can't stop doing it.

    thank you for continuing to provide such informative content! There are always naysayers but those people aren't being vulnerable and working their asses off to help others. You have chosen the more difficult and brave path of having your face, name, and voice behind a project that discusses such a stigmatized diagnosis. Content like this helps people understand themselves a little better, and hopefully have more compassion towards themselves on the road to recovery.

    Overcoming my Disability has been challenging to say the least.
    I was off work for about a year. Working from home ruined me.
    I am back to work for the last two months at a new job that is supportive and understanding where I can work on-site and easily separate home and work.
    I am learning to manage my symptoms and am being given the time and space to work at my pace, and by doing so I get to help other people as part of my job.
    All I want to be is helpful and useful to others and my team at work.

    I have to agree with this. Treatment centers usually become an island of peace and quietness and it can be healing for some people in the beginning to take distance from the triggering circumstances... but in the end it teaches them nothing about how to solve the issues that took them there to begin with. It is also not rare to see people come back one time after another because they would rather stay admitted forever in treatment centers than go back to their reality. That is not well thought-out.

    The word trigger is over used, particularly by teenagers. The word needs changing in regards to people with BPD or DID and other illness.
    Many of us who suffer know that triggers are the catalyst to a change of mood and personality.

    How do we avoid work that is triggering? My job is so triggering but I have to work. And finding another job has been impossible. How to avoid a spouse who is mentally and emotionally abusive but you can’t support yourself financially? I’m at the end of my rope because I can’t cope with either of these anymore.

      I’m not doing that anymore. I want to be comfortable. I do avoid people and situations that trigger me. I see nothing wrong with it. I am in a cave and have no mate or bestie. I feel empty and alone.

    Pls do more videos of un expected or seemingly random nature events that ruin peoples lives . For me I was victim of a unprovoked assult out of no where . I dont know how to make it not happen again other than living in a way that i control absolutley everything down to the last detail . This produces what you are talking about isolation ect . Thing is I cant get hurt like that again, i lost everything my job ,socjal circle ,house ,career ,possesions ,girfriend ,status ,presupositions,axioms ,belief systems. I lost it due to randomness or unpredictableness of life .

    Thank you for these wonderful questions and videos. I absolutely adore Dr. Choi-Kain! I just question this idea (if my understanding is correct) that someone with BPD can have corrective experiences without the therapeutic process or something similar in a real-world context. From my experience, if those with BPD are known to often misinterpret interpersonal situations due to a negative skewing perspective and differing neural activation, then experiences take on the pattern of BPD thoughts or responses. So it drives you further into triggered behavior or avoidance. So, I dont know how you could do it on your own even with the best intentions. You need the other or something similar to provide feedback and a means of seeing the grey in situations. Marsha Linehan in her memoir was lucky to meet several people whose approach as human beings became corrective in this way. So, it's a choice that then requires support. Additionally, treatments can be inaccessible due to fit or cost, and when you really want/need help, it's hard to then convince yourself to pull yourself up by your bootstraps. Especially given that some with BPD show executive function challenges and when triggered all of us can show deficits.

      This is a great comment, so thank you for this. You CAN have corrective experiences that maybe stand equal in impact to treatment - ultimately in the life you build (with good guidance - and yes, the cost often comes into play with that). And ultimately, the life you live outside of treatment is going to be the great teacher and love-giver. We will be doing a series that to some extant addresses this very thing. I am not going to say more until we've filmed some of these pieces, but your comment bears well in that it can help function as a guide for us in those videos. But, to your point, to know HOW to navigate, no doubt many of us need good guides in the form of awesome therapists/healers etc... -R

    Yeah, overall I think this was a FAIL. This really didnt tell me anything new...I know it failed to identify particular triggers which people with BPD working alone are discovering. As just one example, REJECTION, and therefore, there was no discussion of how to deal with that particular trigger. Now, I understand in a video that is 8:56, you cant discuss ALL the individual triggers, but not even one is given, instead what we got was some analogy related to skiing. It's Disappointing.

      Awww... Thanks, Dylan, for coming to my defense. I've developed a thicker skin since starting this channel, but yes, I did read this and of course, I have a barrage of thoughts and feelings surrounding it. And, the comments you make on diff videos and (dare I say) vulnerability in them are noted and appreciated and thank you for sharing some of your thoughts and feelings.
