For example R3(1sc, 2sc in st) x 6 (18) not sl ------ does that mean that you count the first single crochet with the stitch marker and the other sc to the total 18 so like :: 1sc first stich (count 1) , 2sc (count 2/3), 2sc(count 3/4) .etc or do we exclude the 1sc so like: :: 1sc first stich, 2sc(count 1/2), 2sc (count 3/4). etc Sorry I'm new to crochet so I'm still getting used to the counting.

Yes I understand that. If you look at your graphics on screen it states dtr is done before tr & surely this should be the other way round so that the dtr is at the highest point along with slip stitch.
