This was some seriously other worldly stuff when I first heard this at 12 to 13 years old. Seriously, in regards to industrial, I went from NIN to Coil. And NIN sounds like sunshine pop compared to this. Coil and Throbbing Gristle took me to musical places I've never visited before.
It's "fantasizing about being sodomized in Tangiers" Jhonn often said it before playing it in concert, it's even in one of the live recordings here on yt. He never mentioned Burroughs directly, but it's definitely got something of his work in it. There's nothing intollerant in what Alex Harvey wrote, sexual (and homosexual) themes are a very important part of Coil's work.

I always liked coil, although I initially heard about them because I love Hellraiser so much and heard they did the alternate score. When I heard it I was oddly underwhelmed, to be honest. Then I happened upon Horse rotovator...and I couldn't help thinking 'why the hell wasn't this used in Hellraiser?' The album is great and seems to sum up the film so much better than their intended score. Maybe it would have overwhelmed the film slightly I suppose...

Some others in the comments have theorized it's a synth-produced sound, accordion distorted heavily enough that if you heard the original recording you'd have no idea it was an accordion, or (most likely) hurdy-gurdy.
Falco Lombardi chances are nobody but the producers would know. it's probably some contraption of their own design, or an instrument with so much distortion on it that if you heard the sound of the original it would sound nothing like this
Hi I'd say the sound is created from an FM synthesiser, likely a Yamaha DX7 or something similar but with a shed load of delay/reverb put on top of it, hope that helps!

I think it's really just up to you. The lyrics are so mystical that it can't be really said with utter certainty what it is about. Just listen to them and let the imagery come to you. More times than often I am a bit disappointed with the "official" meaning of the song tbh, like with this one

I have this lp and it's still my favorite Coil recording and a complete album experience. Has stood up well over the years.
