It was designed by Fairy Wool in the Wood on Etsy. I went to her shop and it is not listed. I googled it and found it on 123 Stitch, however, when I went to grab the link, it was "no longer available." I wonder if she contacted by the company for copyright infringement? Sometimes that happens. I don't think I still have that chart, but will look. I'm happy to pass it along if I do.
