    I had my upper All on 4 done 2 years ago! So pleased with the look Riz Seyd did . Last Monday I got him to do my bottom All on 4 :). I’m really bruised still but happy. My smile is complete. It’s made me look so much younger. Now I have the perfect smile. Removable dentures were an option. But for me it’s a massive no! To me All on 4 is the way forward. Loved watching this video. You explain so well the options for everyone.

      I can relate with you. I had both upper and lower all on 4 done here in Australia. I'm very bruised and recovering slower than usual due to the fact that I cannot take anti imflammatories but am so happy with the result of the aesthetics. My boys saw me smile after years of hiding behind my hands :)

    I have had about 5 consultations with various dentists. I hesitate because of the cost, the anticipated discomfort, the process with fake dentures and lack of trust. This will cost a lot of money and I don’t want to look crazy after having it done. I also don’t want to look as if I’m wearing dentures. I’ve learned that we don’t all have the same understanding or expectations when it comes to aesthetics. Thank you so much for this video. I appreciated your calm and clear explanation about the options, some I was not aware of. I have a decision to make in the next 3 weeks so this was helpful.

    The whole idea of screws in my jaw makes me cringe.
    I really thank you for this information!!
    I'm fighting dentist to just stop "selling" implants.
    My thing is, I'm ok with removable dentures. I keep hearing that the dentures will "rock."
    I've already spent most of my life picking and choosing which foods to eat.
    I thank you, heartily for confirming my situation. Cost is a barrier, but most importantly...I honestly, do not want to continue fighting dentation forever!!
    Plenty of ppl I know do just fine with removables.

    Thank you for this video it has been very informative and has answered many of my questions... on the other hand you explain everything with a soft voice and that transmit a sense of calm like saying... "It's all right, don't be afraid, everything is going to be alright..."

      happyfamily1004 hi! No, I don't, allergies to titanium or titanium oxide are so rare, I never recommend this. I have however recommended it when I had a patient where the implant didn't stick and I didn't know why! But even in this case, the test didn't find anything.

      Great news, thank you! Dr.Gurs I can't decide to choose between Zirconia and Phonares by Ivoclar. My dentist offers me the same price for both of them. So, financially it makes no difference. Which one do you recommend?

      Personally, I would go for phonaris acrylic wrap- is easier to fix in the future and it has a better shock absorbing properties. They can both look amazing if you use a good lab technician!

    If there's a better video or web page to help people understand the options available I haven't found it. I spent several hours searching for information to help me decide between a removable or screwed in hybrid denture and couldn't find anything close to your video. I did learn elsewhere that with the hybrid you must be prepared to be very diligent about the cleaning of the implants to avoid bacterial infection and bone loss and this speaks to choosing the removable denture simply because maintenance is so much easier. I'm still not clear how it's possible to immediately load the implants. I advocated for this solution but none of the dentists would commit to loading before the implants had at least 4 to 6 months to stabilize. Again, thanks so much for this terrific video.

    I am shortly looking at removal of all my teeth (in 3 days! Yikes!)
    the all on four with the implants with the bar seems the best option for me.

    Thank you for making this very informative video to sorta give us a ballpark figure of costs. I had my top teeth extracted yesterday and have a denture and have been thinking about snap ins or a more permanent solution and your video helps me realize my different options.

      I am about where u were in my process I only had 11 teeth left on the bottom
      I had the rest removed When Hurricane IKE pounded us here in Houston. So if u have a minute can u tell me a little more about your process was it successful?. Are you able to eat what you want now ?

      Thank you for your kind comment, there are several options available, but it sounds like your'e doing the right thing - get everything stable first by removing or repairing the bad teeth, then you can look at improving the dentures.

      Look at corporate interest in Dentistry is significant. Profits on implants are massive. !!! It will plummet in a few years.

    Thank you again for so much valuable information. I am going to travel abroad initially for a consultation. My lower partial denture is awful on a practical and aesthetic level and eating is a nightmare. Missing upper and lower teeth and those I do have are not attractive for several reasons. I know I have very little bone so options might be limited and being in my late 60s I consider looking as good as poss without too much invasive work, might mean more but better fitting dentures. I just dream of a lovely white smile and having a social life again. I couldn't afford any of this in UK but could abroad, so this is what I have chosen. I'm terribly dental phobic but I know the expertise and state of the art equipment will ensure I will receive good treatment. My NHS dentist has no interest in doing anything to help me. His priority is to do the minimum work, never discuss my hopes/wishes and get me out of the door. I always feel embarrassed and humiliated when I have to visit. Many thanks, you are obviously a very caring dental surgeon. 🙂

    Thank-you for your videos. I am in the process of looking at what my options are. I am a relatively young woman (63) with a family history of people living into their 90's. I have had removable dentures for about 1 1/2 years and know that my next step, which will make life more enjoyable for me is implants. I have been told that I will need sinus lifts and bone grafting. Thankfully my jaw is fine for implants. Your videos have been most informative and helpful as I start this journey as they have helped me understand what the procedures are and what the different options discussed actually are. Your videos are excellent in explaining all of this to me and will help the decision-making​ process all that much easier for me.

    So the last one, the 4 in one implants… Do they still have the gums that push your lips out and make your lips look thin?
    Is there something new called G4 and you don’t have to have those fake gums?

    Good video. As someone who had 11 implants placed (bridge top, arch on bottom) I do not understand how such a process can be done in less than a year unless the patient is a healthy 20 year old. Even 1 year would be extremely fast.

    I had mine done for the past 5 years, 50,000 the tops do not fit. After year of precise measurements still not right even with snap on 4 screw with titanium bar. How is that not possible? Bottom goes over my gum food sticks that water pick won’t help. Inner is gap my tongue feels with even 4 screw down perm. I’m just torn . Then back bottom broke off. Made so incredibly thin no way it would have stayed.. Lab says was made perfect and material s were top brand. No suction...ugh I can’t afford to pay for more and more fees. I just need a set made. I have all work and metal. Any thoughts?

    Hi I had 8 implants done in Turkey in October. They did a sinus lift and bone grafting but left one bottom back tooth on left and 3 on the right and said it was for the temporary dentures. They have fitted NTA Implants and I’m supposed to be getting ceramic teeth. The denture they gave me does not fit at all and so I’m toothless till March when I go back to Turkey for the permanent teeth. This was the fault of the agency messing appointments up,my sister brought the temps back with her so they were not tried. The teeth left are really cutting into my top gums,will I be ok to have them taken out and will an nhs dentist extract them ?
    I have tried and tried to discuss issues with the agency but seem to be getting nowhere. Are NTA good implants ? Many thanks Christine

    Very clear and helpful. I have wonderful expensive implant dentist and fortunately I can afford it but two funny things he says when discussing he choices "let's take the money out of it" and "do you know that the people of Ecuador spend twice as much on their teeth than the bloody English . He's right

    One of the largest factors in heart attacks is gum disease. Bacteria from infected gums is continually entering arteries and being attacked by the immune system causing plaque and clogged arteries. Along with apperarance and function, healthy gums should be an important factor in the selection of dental processes.

    I am on stage 2 for the bottom bar (implants placed, gums have been opened, now I have about a month of healing before the bar is placed and the snap on given to me), and a removable on top. Honestly, the top removable has never bothered me. But the BOTTOM that actually does the chewing, I could not deal with a typical, unsupported denture on the bottom. I paid 15k for the bottom and a new top removable (got a bit of a price break on the top removable since we did the surgery, a temporary denture for healing, and a permanent snap-on, all at the same time.)

    Thanks for explaining dental implant options in details. I read somewhere that implant doesn't work well with people having history of gum disease. I'm in my 40s and lost teeth due to swollen & bleeding gum infection. Should I go for All on 4 type implant? Thanks.

      Hi, yes, dental implants placed in people who have gum disease are very likely to fail early. It is important to address the gum disease first, then have implants. Something like the all on 4 will remove the gum disease as soon as the teeth are removed, but in those people who only have one set (either top or bottom) treated with all on 4, it's very important that the other arch is treated for gum disease.

      I know this is a old post, and you probably already have your answer. Your jaw bone, and gums will continue to shrink until you die with no teeth or implants to hold. This is the reason for ill fitting dentures.

    I have osteoporosis which is affecting my teeth !!!! I am so confused about what I can or we be able to do and when ??? This has helped me understand a little more ! Thank you ! If you could give me some advice , I would greatly appreciated it !!! I have cried my self a sleep over this !!!!!!!       This is a really important topic for a lot of people, and massively effects what can be done with dental implants and surgery. I will add it to the list of videos that I have planned... As a summary, you just need to keep treatment as simple and straightforward as possible, because the rate at which things heal is often less.

    Thank you for the video it helped me understand the different type of implants. I'm looking into them because I have a full set (both) but the bottom always becomes loose no matter how much inhesive I put on it so I need something a little better. Does it cost more if you have a tiny mouth

      Hi, Mike, there should be no difference in the cost if you have a smaller mouth. As long as the required number of dental implants are placed, then the size of the mouth doesn't matter.

    Tell me Doc, is it possible to have the mini first, then after a few years, if everything is fine upgrade it to bar ( without removing the minis ) then ultimately ( again without removing the same minis ) upgrade to permanent bridge?

      Hi - good question and I think it depends on the implant brand. In general things like bars are designed to fit on standard sized implants rather than minis. A better way might be to have standard sized implants first, maybe with locator attachements, then you can fit a bridge. The best is to go straight for the bridge, but I know this is not suitable for everyone

    Hey Doctor I have a question about my lower dentures, with two mini implants in the front. I really like the way they fit and look so natural, but when I even just press down firmly on the rear my left side of the lower it is to pain full to wear them to eat. Would two more mini implants in the back relieve this pain so I can eat or what options can I consider. Thanks in advance.

      Hi Ted. The answer is - probably. If you have two more mini implants, that means the denture will be fully supported by implants. Like having a chair with 4 strong legs, it should be much more stable. Right now, your denture is sitting on two solid implants, and soft gum, which is why there is an element of movement.

    Thankyou so much for this information. I had a molar that I was having root canal that had a calcified root. My dentist suggested I should get an implant for the tooth. Though he didn’t offer the service himself he had done a lot of reading on it, and had the impression that placement was the key ( I’m assuming the implants integration with the jawbone). Is placement still key with the suggestions you have outlined, compared with single tooth implant?

      Yes, to be honest, most problems with dental implant are not problems with integration, they are cosmetic issues that the patients were not expecting. Most of these problems can be elimintated with perfect placement.
      This is why I normally use guided surgery for single teeth at the front of the mouth.
      There are guided systems for all on 4, but I have not used them as you do not need to be as precise, and there are different guide systems to help get ideal placement for an all on 4, or full arch treatment.

    The end all of implanta is the 3-on-6. I would look into it.
    6 implants on top and bottom, 3 separate bridges. You keep your natural gum line.

    Hi there! I have periodontal disease but it isn't active anymore and is asymptomatic but I have lost some bone. I want to get implants because for me the most important thing is to not have my palate covered, and stability to eat things I want to eat, like nuts, sandwiches, etc and to talk without my replacements moving around. Can 3 implants on the top and bottom be enough?

      Hi! As someone going through this process at the moments(4 days postop of the all on 4) I would say unless you are doing mini implants, I can’t imagine they could do less than 4 on top and 4 on bottom and keep it super tight and stable. The last thing you want it to put too few implants in and they can’t handle the weight of your bite and it causes failure

      Hi Anthony, most dental practices who offer dental implant treatment will have payment plan options, and many at 0% of around 12 months, with or without a deposit. Most of my patients will take this plan for these kind of treatments

    My all on four has failed. My top four teeth keep dropping off or three teeth looked sheared off at the acrylic gum line. It broke off around an implant. Shouldn’t I have been able to see if a metal bar was within my acrylic gums since the breakage was so high up? How do I really know if my prosthesis has a metal bar that is meant to provide strength?

      Hi Sheila, your dentist will be able to tell you what your teeth are made from. If the teeth sheer off like that, then it usually means that an aspect of your bite may not have been fully addressed. Luckily, its normally quite easy to fix - at least it sounds like your implants are okay!

    I have had dentures for a while now and I don't really like it. I know I am due for a refit especially the bottom which has always been a problem for me. This was really interesting and I think I may see how much the mini implant ones are because I can't eat with them (dentures) at all, can barely drink, if they are in longer than three hours I'm in pain. I have a terrible gag reflex and I can't stand the adhesive. I don't know if I can afford the mini thing for clipping on dentures to if it isn't on the NHS but I would probably want to save for them if my bone is suitable. I don't like what is happening to my mouth either as I don't have many wrinkles... I am 60 and the thumbnail was taken about a month ago but, the corners of my mouth are beginning to get downward slanted lines and I hate it...my jaw is losing definition too but I think it's because of the dentures rather than something that would have happened if I still had teeth. I am genetically not prone to wrinkles and it's really disappointing that the teeth or lack of teeth is causing this. I think if they didn't move and cause pain I could leave them in all day and start perhaps to live a more normal life again.

      Foxiepaws ACAnderson - We are in the same exact boat. I thought I was the only person suffering immensely from dentures. They are painful, I hate the feeling of denture cream and no matter how much I try, eating is next to impossible.

    Hey there. Thank you for the video. Got to learn a lot. I have a partial empty mouth. My teeth were extracted due to severe neglected periodontal disease. The full implants failed due to receded bone and the bone graft was also a disaster. Now the right side (healthier) is loosening up. I want to get them all removed and want something fixed. Do suggest me something. A solution please. I can go for the temp ones too but I am afraid of the low chewing capacity.

      Sumeera Hassan I would go to someone very experienced, and do everything one stage at a time. So bone grafts, wait 9 months, then implants in, wait 4-6 months, then have teeth made. This could be a 12-18 month project.

    I am fortunate to have a dear friend sponsor my solution to my dental nightmare. I went to see a dentist who was offering to do an all inclusive dental bar, but only after watching your videos do I understand what they are actually! For over ten years I have, due to financial limitations and a level of poverty, had many decaying teeth pulled and at 64 years if age, am down to literally only 16 teeth (one with a temporary cover over a root canal.) After almost committing to getting the bar uppers and lowers 4 in all, for two arches, I was scared off by the disclosures the office manager made, warning of the possibility of 1) permanent numbness in my gums due to 8 hrs it would likely take for the surgery and 2) severe bruising that was highly likely 3) the risk if death (as I am a recently diagnosed diabetic) while under general anesthesia for 8 hrs. I am scared and weigh the pros and cons of that! Plus the cost even if my friend is willing to pay for someone with a potential life span of maybe 25 years max! Your thoughts please? I wish you were in my town! I would choose to see you, without question !

      No, the bar sits just above the gym by something called an abundant which connects this bar to the actual implants

    With the fixed bridge option can't bacteria still get under the teeth. Like in the days of bridges done with your own neighboring teeth acting as abutments? Thanks.

      This is possible, but if you're treatment has been planned well enough, then it should be quite easy to clean the bridge with either super floss, water picks or tepe brushes. The fixed bridge should not rub on your gum at all, there is no movement between the bridge and your gums.

      @@drgurssehmi but the abutments, which were my own real teeth got pulled out somehow with the bridge. So I'm told they cant put the bridge back. Well he did but it came out right away.

      Hi from when i was a Yung child my teeth were the colour of a 40 a day smoker first my milk teeth them my main teeth same, by the age of 13 had 6 crowns up and down but they well they did not last and are in need of a refit i don't think i will ever like my teeth and i do not see me having the money to afford to replace and the other work i would need and i am not going to be able to keep standard Dentures in my mouth , thank you for the videos as ever kenny

    I'm now.learning what I'm gonna do. My teeth are just about done. I want secure and permanent teeth that look real. Question for you is I smoke cigarettes and can I still brush and clean will they hold up ?

      Smoking definitely reduces the success rate. So there is an increased risk that the implants will not stick in the short term (in this case a new implant can normally be placed), and there is an increased risk of gum disease around the implants in the long term - this is a much more complex (and expensive!) problem. I would highly recommend to stop smoking, but I know thats easier said than done!

If you are still doing I can always come to you if that's an option ? My local dentist didn't sell me on confidence he knows much about implants

    Interesting, honest and informative. I'm trying to get the work done in India where I can afford it but info is hard to find. Is it true that all on 4 is just an implant supported denture with a smaller flange and a bigger price? Can you have all on 4 on one arch and ISD on the other? If I have badly receded gums would I be better with ISD regardless of price? Cos of smaller flange can't you see the edge on all on 4?
    I hope you get the chance to address these questions. Or better, offer me Chennai price in London and I'll stand lunch at Saravanah Bhavan.

      Hi Mark, some great questions. So the all on 4 bridge shouldn't have any flange, this reduces the food trapping. It is made on a CAD Milled metal bar, and this is significant in the final cost, where as the ISD is essentially an acrylic denture - much less expensive. During the all on 4 treatment, there is normally some bone reduction to hide the edge of the all on 4 denture behind the lips. You can definitely mix the ISD with the all on 4 bridge. It is difficult to say what would be better for you, in many cases both solutions are possible, it depends on if you prefer to have something removable or if it needs to be fixed in position, and how much you want to spend.     Interesting and VERY informative. The only thing I would strongly dis-agree with? is when this man says normal 'dentures' are ok! THEY ARE NOT! In my personal opinion? they are the PITS! Until this man has 'had' to wear dentures? then he really should not give an opinion on how acceptable they are. The simple facts is, to prove what I say is true? is the mere fact that he is making this video in the 1st place, because it is because dentures are truly as bad as I say (and why an awful lot of people are finding out about what else is available and possible), is all the proof 1 needs. But as always, its the 'cost' and as always is? will un-doubtedly discount a huge majority of people who simply will not have any option but to walk away.

      What you say is 100% true for you, and something that will resonate with a lot of people. I know that it is not the case for everyone - many of my patients who I follow up with regularly are very happy with removable dentures and no implants. Implant retained dentures or bridges on implants are much better - its all about finding the right level of treatment for each individual.

      The problem with making sweeping declarations as you have is that they aren't true for everyone.
      I had massive pain and expense trying to save my remaining teeth. The dentures have been a god-send. A year after receiving the first set, I had new ones fabricated and the fit is phenomenal. Another couple of years and I had another set fabricated.
      My confidence went up, my job situation improved because I smile confidently.
      Sure, there are issues; but my life is so much better with dentures than with failing teeth.

    Had mine for 2 years and only eight teeth have fallen out. What do you expect when you only pay 48000 for extractions, temps and permanents. My experience had to vent.

I went for a few consultations and each time, I was informed a full mouth would run between $40,000 to $50,000. One would think that by now, the prices would drop. Thanks in advance.

    Denture are rubbish having to deal with adhesives also u can’t bite into anything alleays coming backwards and forwards to the dentist and in time u loose ur gum bone so ur dentures are no good
