It's usually when the player plays a move that raises issues for their opponent, i.e Hikaru is smart enough to see that a certain move would cause for example problems in the endgame, or cluster his moves, and so on. It's like spotting a move only the computer would see right. It's high-level shit.
The quality of a move depends on how much the move hinders or pressures the opponent. At high level each move does so much that another high quality move is required to stay in the game. Hikaru was making high quality moves but he got suspicious because none of his mattered because he got outplayed which based on his rating wasn't reasonable to Hikaru.
