He did use his throat to scream, yes, which caused his vocal chords to crash together and tear at each other. But he never took care of his voice, and he pushed it. I scream exactly how he did in CYB, I sound almost identical, but I take care of my voice, now. Before, I had the same shit happen with my vocal chords, and I used my throat. That was two years ago, and I messed up my voice. Still pains me today, but not as much, as I've healed it, and I can mostly hold a screaming session for 2 hours.
I don't listen to deathmetal very often, but when I do, I can't understand ANYTHING that the singers are saying. Is it something you get used to, and gradually understand better, or do you just stay oblivious to what anyone says?
