A homicide detective goes undercover as a patient to investigate a psychotherapist he believes is linked to a strange double murder. As his therapy sessions continue the line between fantasy and reality begins to blur.

Can somebody explain to me the full plot ? I can't fucking understand. Is he really a detective ? Or a patient ? Or a detective that is slowly tricked into becoming a patient ? How did the psychotherapists survive the gun shots like how they explained the crime scene in the beginning ?
I have to admit I have to skip a few minutes, because I'm not sure when the realities switched ???
