There's a bunch of stuff from the manga that this OVA omits, which could take a while, but other than some relatively minor details (Ikuro playing banjo during the crash and nearly drowning in ice water as a kid, Doress being around since WW2 and under the command of Douglas MacArthur after Japan lost and their lab being under a Buddhist temple with a statue that read your retinas for access, Notsuo being able to inflate his tail like a balloon to fly and having retractable spikes in his knees, generally more explanation given to how the Baoh Armed Phenomenon works and inner monologues and of course "BARU!" etc.), here's the main ones: 1. In the manga, there was an old couple who take Ikuro and Sumire in as guests (hence the title "Baoh the Visitor") and Sumire lives with them at the end. The old man, Rokusuke, was also hypnotized by Dorudo (the cyborg) into shooting Ikuro, but it doesn't work. They also have a son and it's sort of implied that they're Ikuro's actual grandparents. 2. The manga also has some more mutant animals for Ikuro to fight: a giant mandrill made by Doress called Martin with weapons in his stomach, some bats called Aroma Bats with weird breath that screws up Baoh's sensory organ (the yellow thing on its head) and giant spiders called Sap Spiders that drool acid.
There's not much to continue. I read the manga it was a short series and the anime did a great job to cover all of it. There's a few differences like a few more battles one is with a beast tamer and the beast which was good and that girl who died in the cave with the professor she died earlier in the manga.
