I think the fact elliot was sexually abused multiple times as a kid as elliot writes about in their book might play a part in it. Making yourself less attractive keeps you safe . Elliot may even be doing this subconsciously

Consider:
Doctors should not be permitted to swap people's right and left arms because those people feel like they should actually be right or left handed. This is because even if such an operation was done as successfully as is possible, it would still lead to people's arms facing backwards, thereby severely hampering the normal utility of both arms. Sure, there might be a few rare actions that would be improved by such a configuration, but after millions of years of evolution, I'm going to go out on a limb and guess that overall they'd all be a lot better off leaving their bodies as-is while considering themselves to be either uncoordinated lefties with incredible right arm and hand control, or uncoordinated righties with incredible left arm and hand control.
