    "Lumbar extension is bad if you have a diagnosis of lumbar spondylolisthesis."

    If we are viewing this statement through a critical lens, we’d probably want to ask some questions.
    How much lumbar extension is bad? 1 degree? 5 degrees? 20 degrees?
    What if lumbar extension actually makes your symptoms feel better?
    What if you have a diagnosis of lumbar spondylolisthesis, but you’re asymptomatic?
    Is lumbar extension (in some capacity or scenario) exacerbating your symptoms and prolonging your recovery? If yes, find ways to temporarily reduce or modify the aggravating activities. If not, why is it a problem?
    Several other points from the video:
    1. The goal isn't to fix the presentation of the spine on imaging.
    2. The presence of spondylolisthesis on imaging may or may not be associated with someone's symptoms.
    3. We should not assume a purely biomedical model in which flexion = good and extension = bad. It's never that simple.
    4. The goal is to improve function. We cannot expect someone to permanently avoid extension, especially for overhead athletes, gymnasts, etc.
    5. It was mentioned to perform the exercise based on tolerance.

      Thank you so much for this. I have been trying to learn more about spondy ever since I was diagnosed with it. The surgeon basically said I should wait till my bowel stops and legs go numb, and then we can do surgery. Till then exercise, but how, what, and how much, I was not told anything. I want to return to active sports and high workout capacity. This will really work as a reference guide.

    As a family practitioner, we see back pain as the top five reason people see the doctor. In general people have 2-3 mm of disc herniation in the lumbar region, arthritic changes, and mild scoliosis. And, of course, all of this worsens with time and strenuous work. Another reason people get back pain/problems is prolong sitting in a sedentary career for 10 to 20 years. In my opinion after age 30 or even earlier, we should all commit to stretching at work three times a day and these exercises three times a week. Back problems are at epidemic and we should be trying to prevent these problems. These exercises will probably apply to most back issues. The key approach is to, as he mentioned, start with easier levels, and avoid if there’s any pain. Will be sending this to my patients along with PT referrals if they wish. Thank you.

    I can’t wait to try these. I’m 65 and was told I have this and mild scoliosis, SI Jt Dysfunction, IT Band problems, arthritis…etc.
    I just started PT and the first thing I was told is to NOT lay on my stomach and raise up arching my back. Oooops, I’ve been doing that.

    Thank you so much for the very informative video!! I got so much more of an understanding in this short video of what spondy is and what to do about it than I have gotten from two doctors, two chiropractors, and one physical therapist combined. Had my first major pain sesh two summers ago, minor pain has come and gone, limiting my physical activity since. Hopefully I can return to somewhat normal by sticking to this plan.     Thanks for this video and information. I've been dealing with some lower back issues after doing heavy deadlifts and I don't wanna rely on a belt for my exercises. Hopefully this will strengthen my lower back and core

      Do your spine a favor and wear a belt while exercising. It will avoid unnecessary wear on the spine

    I'm a retired physio with spondy L45. I'd stress to people that they must be able to maintain that pelvic tilt with back flat on floor before progressing to even your 2nd exercise. Also I've found that extension to neutral only over exercise ball for example is the best option. Planks unless you have very,very good core strength is a no,no. There are loads of other exercises you can do that won't cause a flare up the way planks can.

      Would love to hear any other exercises you think are good to try. Been dealing with spondy for about 8 years though walking and Swiss ball exercise mainly. Currently having a bad flare up....hoping to try some other exercise from someone who has been through it. I've learnt that what suits one doesn't suit all with spondy. Always looking for more tips. I'm 56 and hoping to never go down the surgery path. Willing to put in the work what ever it takes. So far so good until recently. I know it will pass but want to throw everything at it. Would love to hear any tips you may be willing to pass on. Thanks for your post

      ​@@tonyfletcher1718 Hi mate, I was diagnosed with spondylolisthesis 8 years ago as well, so I can definitely relate to your comment. I'm 44.
      Yeah, it's hard to find the right movements because we are all different, as you said. Trial and error... it's tricky because I find even if it doesn't hurt in the moment, it might the next day, so adding in movements one at a time seems like the best bet. If I throw in multiple new movements then it caused a flare up the next day I won't know which one was the culprit. Also, making sure form is on point is crucial.
      I still get pain regularly, but it's not as bad as it was, so I'm progressing. This is a slow and steady race. One thing I can share with you is the first thing I did, was correct my pelvic tilt, because this was putting stress on my back. I not only did excersises to correct it, but I also constantly got in the habit of being concious of it while I was on my feet. The way I did this was every now and then I would place my hand on my lower abs as a cue to "pull in" my pelvis to remember to keep correct alignment... if that makes sense. So I'd be walking around, with my hand on my lower stomach lol, until it became a habit! This alone helped a great deal!
      I'll overcome this eventually... it sure sounds like you will too!

      Thanks for the response. Great to hear from a spondy brother. Yeah the pelvic tilt is a good one. Sometimes catch myself in a reflection and see it. I try to incorporate it into my daily activities. Every time I press the button at a pedestrian crossing or cleaning my teeth. Not sure if you have heard of Bob and Brad on RUclips. I've been watching them for years. They are physios. Brad has spondy and gives lots of tips. Some don't work for me but as you know you have to pick and choose. Best of luck with your efforts. I think the exercise path gives you some control rather than giving it over to a surgeon with fingers crossed. I researched it alot in the beginning. I found a study that said if successful(if) pain reduced but 10 years later the pain level is similar to someone who did not have it done. It can be frustrating but I think staying active and helpful exercise is the way to go. Nice to hear from you and all the best 👍

    On a side note: I'm a physical therapist and injured my back when I was 20 squatting heavy. 2 years later I had an MRI showing bilateral L5 pars defects. 3 years later I had another MRI showing a grade 1 spondylolisthesis. By 28, I had the same findings as above plus degenerative changes at L5 and opted to have a L5-S1 fusion. My function and tolerance to running and weightlifting had significantly declined and I believe this may have been avoided with proper management, a good physical therapist, and diagnosis from the beginning. Over the years I sought out multiple opinions and my only surgical option was a lumbar fusion because I had delayed surgery. In young patients, generally

      Same. Diagnosed with Grade 1 Spondy via MRI after 5 years of neglecting the issue. Hopefully with PT I can manage it.

    Thank you for this video it's amazing. These simple routines mixed with cycling and better nutritional habits has meant I can complete activities again with my daughter. I had an accident which caused spondylosis in my l3-5 disc's and multiple damaged nerves. A couple of months ago I made my own stretching programme from this video and Austin Goh's upper back exercises. If your sitting there with these sorts of issues thinking it can't get better give it a go it's now something I do daily.

    What about isometric back/hip extensions for anti-flexion.
    I do them on Roman chair (45 degree) to emphasize spinal erectors. Can be done on GHD, but I find this emphasizes hip extensors more.
    Can be done with 10 second Y raises to strengthen lower traps (also deltoids, rotator cuff, serratus) and to challenge anti-flexion more intensely. This progression is analogous to ab rollouts vs planks for anti-extension.
    The progression I was thinking:
    1. Isometric back extension
    2. isometric back extension w/ bilateral Y raises or Y presses
    3. isometric back extension w/ alternating unilateral Y raises or presses
    I was experimenting with these following a few episodes of low back pain, but couldn’t tell if they were helping or hurting.

    Thanks for the videos, guys! I have found that my mild spondy and DDD symptoms were actually grossly exacerbated by the TMS mind>body stuff since I got really scared after my MRI findings. I spent a few months of catastrophizing and fearing, having my fixation on the "neutral lower back for the rest of my life" thing, lol. Needless to say, my brain reacted to that, so in trying to protect my body from bending, extending and twisting the lower back, it gave me lots of pain. As soon as I realized what was happening, that my pain was actually not structural, but from my scared brain trying to protect me, from that moment on, I have started going back to my daily active life and the pain is almost gone! I have the feeling that lots of the spondy sufferers freak out, just as I did, cuz the thing (the spondy) looks quite scary on the imaging studies. From that moment on, they get lots of flare-ups with pain from the sheer fear and doubt that there is something deadly wrong with their lower back, so they need to immobilize it. In any way: overcoming the fear, core strength, maintaining good weight, proper nutrition and rest, seem to be the key factors, which lead one being out of symptoms and pain!

      What was the degree of your spondy and DDD, did you have a pars defect? I also am freaking out here, limiting lots of mobility in my spine to see if I can "heal" the pars without surgery.
      I'm doing these exercises 3x a week along with mobility/stability exercises. Hopeful that I can make it through this really bad flare up and diagnosis. Pain is really bad for mental health.

If u r stuck in chronic pain, anxiety, depression, stress etc, realising and understanding how your brain works and how you can calm it down, stir your thoughts in the right direction, ground yourself in the present moment and reassure yourself that the end of the world is not coming for you (lol), this becomes a life changer!

      @@anujbhatia3337 the worst thing you can do (I did it too) is freaking out and to start reading all the posts in "support groups" consisting of people telling each other how bad they and their lives are, being constantly in pain and projecting horror stories to each other. Once you have ruled out some emergency condition (which usually comes after serious incident - car crash, falling from a tree, tumbling down a mountain slope etc), that requires immediate surgical intervention, then you can basically work on yourself and the root cause of your symptoms, by calming down your freaked out brain! Please check on RUclips these guys: Howard Schubiner, Dan Buglio, Jim Prussack, David Hanscom, Les Aria, Alan Gordon, just to name a few of them. You will be fine, bro!

    My CT scan said Spondylolysis and Spondylolisthesis of the at L5-S1. I have no idea how long I've had it. Ive had back pain most of my adult life but I have not had much if any in the last 10 years after I started hiking (I'm 68 now). I cannot think of what might have caused the Spondylolysis (a fracture?). The above was first mentioned on a pelvic CT scan back in 2013 that was a routine scan a year after cancer surgery. I had to have been there long before that scan but it was not mentioned on 2-3 previous CT scans. They did not give any classification or refer me to a back specialist or even ask me if it had symptoms. A scan a few years later did say "Degenerative changes in the thoracic and lumbar Spine".

    I disagree with the supermans because these basically result in lumbar hyperextension and worsening of the spondylolisthesis pain. All the other non-extension exercises are good

    I have anterolistic L5S1 spondlyothesis that is degerative and isthmic i was dianogosed at 11. Lying on my stomach with a pillow pushing up and out feels better than down and in it also feels good to bend over a chair for me too

    Thank you for showing different levels of flex ion and extension exercises. I showed to my PT and he had you as a TA.
    What to do when back hurts after bicycling or stand up paddle boarding?
    I’ve tried facet block and it’s not effective in pain reduction.
    Only doing yard work with back brace helps the following day with reduction in back pain?!

    I appreciate your information. I am a recovering from severe stage 4. I am in my 5 week
    from a double fusion surgery. I did everything to avoid the surgery for 3 years. Then there was no more that could be done. Please talk about a routine for recovery and maintenance.

    I've also seen Bob & Brad's videos on this. They seem to be against any planks and many doctors would say don't do anything that would cause you to hyper extend your back (like the superman exercises). Is this old information?

    An extension (Superman) is the worse exercise you can do for spondylolisthesis / anterolisthesis and it may just worsen your situation so I would advise people with this condition to avoid this

Does these exercises work for this condition: At this level there are bilateral spondylolysis of the pars interarticulares of L4 leading to grade 1 anterior spondylolisthesis of L4 on L5 and severely stenosis of bilateral neural foramina which could compress the L4 nerve roots

    I have Spondylolisthesis and am always looking for new workout activities to include.
    This is the only mention of supermans or anything that remotely includes arching the back in such a way. In fact, I’ve been told bending backwards is not ok to do. Also, i’ve been told that deadlifts are not safe to perform.
    I have stable grade 1 ithmic spondylolisthesis at L5/S1 in conjunction with a sacralized transverse process at L5 and iliac crest causing a 7° curvature of my lumbar spine with right hip pulled up / left hip pushed down.
    If you have any specific advice Id love to hear it.

    I have been diagnosed via X-ray with L5 Spondo. My Orthopedic doctor and my PT were emphatic that they did NOT want me to do any exercises that arched the lower back while lying on my stomach, and some of the exercises you show (Superman) do exactly that. I suppose that's the issue with consulting RUclips for exercise suggestions - there are just so many different opinions.

      I have the same diagnosis and I was told the same thing. This is the first time I've seen someone advising exercises like that for spondo, and I just don't trust it. Through all my research and discussions, I've generally found the same advice - focus on strengthening your core + avoid lower back extension, heavy lifting, and repetitive bending over. The thing that has helped me the most is focusing on keeping my core tight and keeping a good posture. I'm still trying to build it into a habit.

      @@CottonCandyNationYT I couldn't agree with you more. My PT has me focusing on core strengthening, and no lower back extensions. In fact, if you'll search for the McKenzie Method on YT (this method uses lower back extension, et al), the certified practitioners offer the caveat that their method should not be used if you have Lumbar Spondy. My PT is also using manual compressions (pressing down) on my lower back while I'm on my stomach. When I'm by myself, I place a foam roller between the SI joints and the area of the L5, and just let my weight sink onto the roller. The stretch on my lower back is awesome.

    I got this in 2020 cos I was going to the gym everyday. I didn't know about good form or rest days. My PT advised me to stop weightlifting and switch to calisthenics. I've been better for it and feel less pain tho it's not completely gone.

    I was recently diagnosed with anterolisthesis of L4/L5 on Xray (most likely degenerative). It's the grinding sound I'm hearing upon spinal rotation that scares me. The Xray doesn't show bone on bone. 🤷‍♀

      I've found that the "dead bugs" exercise is really helpful for anterolisthesis. I'm not sure if this would help as much with retrolisthesis. I try to avoid any "cobra" pose types of stretches.

      I did in an effort to gain lean muscle mass in my late 50's and I had to have surgery to fuse L4 L5. A week after my surgery was the first time in my adult life I woke up and had a full day with no pain. Absolutely life changing. An absolute must is to retain core strength. I'll admit I neglected my core exercises 3 years after my surgery and got a sharp pain in my lumbar region, got back to working on my core and I'm much more comfortable and confident in my back strength. Strengthen by resisting motion in your back. Keep the spine in a neutral position such as during dead lift.

    I have retrolithesis in the lowest vertebrae L5 I am almost 60. Do you have any exercises to help? Thank you! I have been to therapy for core training but they were not real familiar with retro and I was afraid some of the exercises were not good for me.

    Isn’t the Superman supposed to compress your spine way more than is recommended? Am I wrong about this but this seems like it would be pretty bad for what you are trying to fix

      Several points:
      1. As mentioned in the video, the goal isn't to fix the presentation of the spine on imaging.
      2. The presence of spondylolisthesis on imaging may or may not be associated with someone's symptoms.
      3. We should not assume a purely biomedical model in which flexion = good and extension = bad. It's never that simple.
      4. The goal is to improve function. We cannot expect someone to avoid extension, especially for overhead athletes, gymnasts, etc.
      5. It was mentioned to perform the exercise based on tolerance.

    (grade I). Bilateral facet joint hypertrophy is seen. AP canal diameter is 19.6mm.

      I wouldn't characterize spondylolisthesis as the discs slipping, so nothing to reconcile on my end.

    If u do what he says u will be in loop and u will not heal or feel better in tie long run
    U will get relief but u will always chase ur tail
    U hav to attack ur problem
    I suffered for 10 years
    No doctor physio chiro could or wasnt to help
    I took thousands of pain killers
    I found the answer with the grace of god
    It took me 1 week to feel 90%
    Better
    No waynts people to get well
    U hav to doo it urself
    Its hard work and discipline
    Now i know how to fix any back problem except a real broken back
