When Hurrem blames Nigar Kalfa that she'd betrayed and once a betrayer always a betrayer. She did the same to Leo, she betrayed him. The acting in this series are really incredible, from Leo uncomfortable death position, to Hurrem cry with shaking, she can't even let out a sound. U can really feel the sadness from that scene.

It's the factor of "insecurity" that comes clear both in the characters of Ibrahim and Hurrem. Ibrahim turns physically violent and cruel while Hurrem dies it verbally and her venomic stares. When Hurrem blames Nigar Kalfa that she'd betrayed and once a betrayer always a betrayer. She did the same to Leo, she betrayed him. Ibrahim just created the setting , Hurrem completed the betrayal and took the breath out of a man(Leo) who she'd "already killed" to safeguard her power and prosperous life as a Sultan.
If she was clear and clever she confronted Ibrahim in the presents of the sultan with the poison and ask him to eat...
