The reason they lined up is because the British used muskets (equivalent of shotguns), which were less accurate so a volley of fire was the most effective way to utilize them. We Americans used rifles which allowed for more accurate fire. Hence why people in the front lines were not necessarily going to die because if it does hit you it might just be one piece of the shot. Also there were no radios so it was good to keep men close together where flags and horns could be observed, further more there was always melee combat afterwards hence why bayonets were utilized and continued to be used until the Great War.

It got pretty ridiculous. Guns and industry kind of changed how war worked, and it took a while for some people to catch up.

There are rules, but only the little guys get punished if they disobey them. e.g. see the US over the last 50 years.
