    What are the pros and cons of doing the crown lengthening WITHOUT doing the veneers?

      You do not need to place veneers if the teeth are healthy and attractive. Periodontal crown lengthening alone may do the job.

    Not everyone does veneers after crown lengthening. Why does the gum grow back in some but not others?

      There must be 3mm distance between the alveolar crest and the gingival line. From the alveolar crest, there is 0.69 histologic sulcus depth, 0.97 epithelial attachment and 1.07 connective tissue attachment (2.73mm). If the alveolar crest (bone line) is less than 3mm from the gingival line, the body will reestablish the biologic width (3mm of gingival tissue from the alveolar crest to the gingival line). If invasion of the biologic width occurs and there is a crown or veneer margin invading the epithelial or connective tissue attachment of that 3mm biologic width, the body will try to "reject" the "invading material," i.e.gingival irritation, redness, swelling and bleeding. That is one reason why you see some crowns or veneers with swollen, irritated gums. There are other reasons for the gingival irritation, but that is a common one.

    Are you bone sounding to determine the location of the alveolar crest for the maxillary anterior teeth? Or cbct? Or combo of both/something else?
    Basically, I'm looking for how you determine when to do bone augmentation crown lengthening vs just gingivectomy. Thanks.

      Watch the video on periodontal crown lengthening surgery procedure, which I also go through in this video. I take pa radiographs of the teeth, then measure from the incisal edge of the anterior teeth to the CEJ, which will be evident on the pa radiograph. If you watched the periodontal crown lengthening videos, I clearly go through the measurements from the incisal edges of the anterior teeth to the gingival line, then from the incisal edges to the CEJ. I can't go through the entire procedure here, but I carefully explain the procedure in the video.
