6mpg is wildly low if you're not max weight.
This new Kenworth t680 they have me in gets between 8 and 11 everyday. I'm also always under 30k though.

My old volvo had to have an entire head job around 450k miles same model as yours from success leasing i think it was low on compression at the time so you might check compression specs and rail pressure
Few things to look into. A new truck with warranty. Rebuild the motor and aftertreatment, delete it with a fresh overhaul, or get a pre emissions truck. Yes it would limit you away from cali, but why spend up to 2$ a gal more vs staying east? As of now the x15 platform is beating Detroit for economy. You have plenty of choices, mater of how deep in the hole you want to go. The d13 is known to have injector cup issues so that might be part of why your getting 6mpg. Food for thought tho
