54:00 a patient that "says" they don't feel depressed & is showing signs of psychosis/paranoia is probably lying the vignette should say "the physician determines the patient is not depressed", otherwise it should be depression with psychotic features since (hx and) we can assume the patient is lying about their depression merely to end the interview
