good question about CT ratio. if the max bulge of RV and LV are at different levels then TWO horizontal lengths may be taken, each from the point of max bulge, perp to a vertical line down the centre of the spine. Added they give the transverse dia of heart. but line or lines are ALWAYS HORIZONTAL, NEVER OBL
