So when Choso was about to kill Yuji, that ability to 'sense' the status of his brothers kicked in and told him that his brother, Yuji, was about to die. But Choso never knew he had a fourth brother, which is why he had a small melt down and left, because he needed to think about what is going on. He wasn't sure how or when or the details of anything, but he knew that Geto had used him and his little brothers, because his ability cannot be wrong. And that means, Geto is the reason the first two are dead, and why he almost killed his last living brother. Once he reconciled - who are his real allies and enemies - he pulled up ready to go to bad for his blood, ready to kill Geto.

Kenjaku at some point took the body of Yuji's mom and got pregnant by yuji's dad which is why he and Choso share blood (so in super simple terms Choso's father is Yuji's mother). EVEN BIGGER MANGA SPOILERS the specific reason Kenjaku chose Yuji's father is because he was a reincarnation of sukuna's twin brother and that's why yuji was so strong without cursed energy
