This is not a one-pressure point activation to defeat the attacker as it is mostly a set up for other techniques or strikes or takedowns. I have done this on big boys who volunteer for the two arm activation, on each arm, and they go right down to their knees in pain. Once the grip is released they will straight away attack and then it's a whole new ball game. You are right in your thought on how this could roll out without a plan to disable the attacker and disengage. I would love to have the opportunity to fully explain that short clip in the video if you come to my school for a free class. We can all participate in a pressure point attack class for any "what if" scenario.

Plus this is on the off chance they are not wearing a shirt or jacket which will make finding pressure points harder.
