You would remove a worn out male insert and place a new male insert. You wouldn't re-insert the same one you just removed. This is only for demonstration purposes of the use of the tool. And if you do a lot of these locator retained dentures, you will need this tool. Or you can spend all day trying to remove worn out inserts and place new inserts.
