"What was it," you ask me? It was neither the Hunt, nor the Blood, nor the Dream. It was the Insight... the Insight that you've sacrificed yourself and suffered for so very, very long for the sake of everyone else... But I can take it, Old Hunter. I am still young compared to you. My heart and mind are unburdened compared to yours. And when the time comes and I, too, am driven to despair by this horrible, wonderful dream, I will look for a successor to take my place... But for now, I know we must dance. Because I know that even though you long for release, and even though I am willing to sacrifice myself for your sake, you cannot allow me to take your place and suffer your torment... So come, Old Hunter. I invite you to dance this last waltz with me... and then, sleep... sleep, unburdened by dreams and nightmares... Free of torment. Free of sins. Free of suffering. Free of cretins. Free of sorrows and free of tomorrows. Free like the sparrows, leaving their shadows...

"Please brother, don't do this. That's what i hear"
100% correct. This is you and your mentor fighting each other because each one wants to save the other from more suffering. Gherman regrets having to fight you to set you free of the dream you are both trapped in, but he will, as he has for all those who came before us and refused to leave. Because despite his "eternal" suffering, his desire to be free himself, that makes him cry in his sleep, he will still sacrifice himself and free others from this dream.

I'll write some of the lore here, so be wary of spoilers.
Gherman being the first hunter isn't a title to mean he's their commander, he actually was the first hunter, who trained and fought alongside countless others while killing both beasts and people who showed any symptom of the disease. Through some misfortune or a willful pact, Gherman has become trapped in the Dreamlands (or Nightmare, its more appropriate name) as the host of this small sliver of it, where new hunters may come and rest. It is likely that his real body is long dead, just as everyone he ever knew are, and his soul can never move on while he remains here. He has remained in this Nightmare for god knows how long, all alone and unable to find any peace. When you finally complete the story, Gherman wishes to kill you to kick you out of the Dreamlands and free you. The old man, despite all his suffering, will still choose to be the one to host the nightmare rather than have you take his place. The player choosing to fight Gherman instead of letting themselves be executed is to choose to liberate him and become the new host. Neither of you fight for yourself here, but to prevent the other from suffering eternally.

Gehrman was never mad at you for your choice. He was sad. All he wanted was to release you from the nightmare, but you wanted to do the same for him. A choice he never wanted you to make as he chose to stay in the dream. Not because he enjoyed the Hunt. Not because he releashed in the nightmare. He chose to stay to protect humanity and keep the old ones trapped in the dream. He's sad because if you release him, you will suffer the same fate he did, and you will live in agony just as him. And he cannot allow that horrible fate to befall you, nor you to allow it to continue for him.

      I agree, it's a soul crushing theme.
I listened to it many times, revisited it many times. And every time, just like this time again, it resonates with me. It's a theme of an old man, a hunter stuck in an endless nightmare. He welcomes the other hunters, he sees their suffering, he "wakes them up" by killing them, hence the graves in his garden. He mourns Maria that he loved, who's also dead. A doll in her image was made and animated in the nightmare. But she'll never be the real one.

The coolest thing about this song in my opinion is the lyrics. The choir is singing in Latin something like this according to the wiki, but there are some slight disagreement on translation in some places:
    Instance 1:
    Latin lyrics:
    Venator
    Ah mi supra venata
Anima tor feci si vre re mal
    Venare insidiosus
    Mira tenebrarum
Hic sic scitis miserere aeterna (or "miseriae aeterna")
    English translation:
    Hunter.
    Ah, I have hunted.
I did it all to cleanse my soul from evil.
    An insidious hunt.
The wonderful darkness (or "These mysterious nights")
This way I knew eternal mercy (or "eternal misery").
    Instance 2:
    Latin lyrics:
    Venator
    Ah mi sumo vela mala
Anima do res mihi somne restas
    Luna insidiosa
    Mira tenebrarum
Di vises mi miserere aeterna (or "miseriae aeterna")
    English translation:
    My Hunter
Let the evil be hidden (from you)
My life devoted to sustaining the Dream
    The insidious Moon
The wonderful darkness (or "These mysterious nights")
Great One, look upon me and grant eternal mercy (or "Great One has looked upon me and grant this eternal misery").
    Instance 3:
    Latin:
    Venator
    A mi sumo vena mala
    Anima doles mihi solve recta
    Luna insidiosa
    Mira tenebrarum
Di vises mi miserere aeterna (or "miseriae aeterna")
    English translation:
    Hunter
    Take my sickly vein
My soul hurts, free me, rightly
    The insidious Moon
The wonderful darkness (or "These mysterious nights")
Great One, look upon me and grant eternal mercy (or "Great One has looked upon me and grant this eternal misery").
    Instance 4:
    Latin lyrics:
    Venātor abyssum venārator
    Tū nōlī at esse supplētūrum
Supprēmum sic falcem quatiam semel modo
    Mē parti si finem praebētō
    English translation:
Hunter, who hold in awe the abyss
But you are never supposed to be a scapegoat
I would swing this scythe once again, to offer an end to your nightmare
But please, give me an end to my role.

I'm 99% sure it's the second translation, but instead of "mira tenebrarum" it's "luna dolorosa," as the Moon Presence is the one behind Gehrman's torment in the dream, and it's cursing it. It continues the phrase a bit better than "that wonderous darkness" to be honest, as he's refusing to let you take his mantle.
Edit: It also sounds like "Luna Dolorosa" right after "Luna Insidiosa" when I have headphones on, and all the other translations have glaring issues where the lines don't match up with the audio

Gehrman, The First Hunter is one of the best tracks ever, full stop. The story behind it is sorrowful, but also Gehrman was responsible for some of the events that led to the Old Hunters DLC. Overall, amazing music for a great fight. You almost feel bad for choosing not to just let him kill you, but you know that whether you take his seat or fight the secret boss after him that you are releasing him from a sentence of constantly seeing hunters trying to escape, something he cannot really do on his own. The choir just takes this from great to amazing and kicks in at the correct time in the fight. It's like the collective group of hunters that have come through the nightmare's shop are collectively crying out to end the misery of the hunt. Your analysis of not wanting to fight is actually spot on too, cause one ending/option for the Gehrman fight is just to let him kill you in a cutscene, completely negating the need to have the boss fight at all.

I also suggest "God Devouring Serpent/Rykard, Lord of Blasphemy" from Elden Ring. This one, to me, is one of the best in the whole soundtrack. It sounds extremely vile, wicked and pure malicious, while at the same time imposing and horrifying, having great strings and a strong choir to express these ideas. Something, that as a cello and violin player who played in a small orchestra, I appreciate to the fullest.
I'd love to see a video on these, one day.
Either way, keep up the great work! It's heartwarming to see a channel sharing with the world the same passions as I do for music. Before I found your videos, I thought I was the only one who overanalysed music, searching for a story, ideas or feelings in them based on sound alone.

Coming here to add some of my thoughts about this amazing piece. The strings section has always reminded me of a conversation between the Hunter and Gehrman, the former with the lower tones (viola/cello) and latter with higher tones (violin etc). It feels like a back and forth between the two Hunters, each trying to convince the other to stop, to not fight, wanting to end the other's suffering and release them from the dream.
    0:00 - 1:20 = Gerhman
    1:20 - 1:46 = Hunter
1:47 - 2:05 = Gerhman (undertones of Hunter making their argument)
2:06 - 2:23 = Hunter (undertones of Gerhman making his argument)
2:24 - 3:04 = clash of both Hunters, argument escalating
3:05 - 3:40 = the Hunter pleading once more to Gehrman

The part @ 4:53 you can even hear the choir just screaming and exploding into this pure agony, sorrow and anger... solidifying the idea that the music is just fitting for the sentiment of Gehrman doing something they don't really want to, but still can't help but try and see it through, to end our hunter's dream. Yet the theme has this triumphant undertone befitting the first hunter.

What I love about Bloodborne's OST specifically is the slow, cinematic build up in most of the tracks featured. It's very different compared to other game sound tracks cause it doesn't immediately go into the intensely bombastic, and in your face sound right off the gate. In some cases not even at all. Two of my absolute favorite examples of the perfect build up are the tracks: "Amygdala" and "The Watchers".
The build up to me is what makes the atmosphere of these fights feel so much larger than life.

"It always comes down to the hunters' helper to clean up after these sorts of messes."

I love the way it starts out in a sort of cold sadness, as if the character involved has lost their capacity to even care, then it gradually develops into a more overtly dramatic outpouring of emotion to try to break through that. Then, at the very end, the final chord again hearkens back to that cold and sort of stern sound. (To me, anyway)

    "Fighting a friend"
For it comes from a person who didn't play the game so-I assume-does not know its lore, yours is the best fitting description for Gehrman's bossfight. One of the most truly moving moments in the entire Soulsborne series.
He's not a real friend to us, but in a certain way he's the closest thing to a friend you can find in the eldritch, wicked world of Bloodborne.

Considering the fact that the choir kicks in once Gehrman starts channeling Flora into himself (Flora being the Moon Presence's name prior to her becoming a great one), your analogy of watching as someone close to you morph's into a creature that has to be stopped is very close to what happens.
The metaphor about coming back to your home after years of being away and finding it in ruins also fits. Either you canonically get kicked out of the dream - the hub area - upon losing to Gehrman and can never return to what became your home, or Gehrman is freed from his eternal suffering as the Dream's host - which means that your home lost its "father" figure so to speak - and he is sent to a world where his physical body likely has died due to him having been trapped their for at least 40 to 50 years, if not a century.
