  • The difference between atrial fibrillation and atrial flutter originates from the source atrial depolarization and subsequent pattern of ventricular conduction.

    Though my attention was mostly on the edit instead of the info, i still caught the difference between the two arrhythmias.

    Yessss. Thank you! I use all your vids to help prepare my new tele techs prior to the class I teach. I also recommend you to new RN grads who want to perfect their rhythm interpretation. I always tell my peeps-The atria sets the pace-With atrial flutter the atria are pro-fusing better, so the ventricles can better regulate their response- so it’s usually regular. But-not always. You can have a irregular(variable ventricular response) rhythm with flutter as well. Is it a 4/1, 3/1, 2/1 block- is the ventricular rate regular, if not-please state that it’s irregular. Is the atrial rate regular? You should be able to state the atrial rate if you can count it-if you can’t count it...
    the rule I tell my students-If you feel like you wanna call it flutter but you can’t define and count count every flutter/pseudo p-wave-just call it course afib... 🤗

      Stephanie, I love that you mentioned the potential of variability in atrial flutter because it's often a point that confuses many of my students. I had mentioned this very briefly in the video I made for atrial flutter (ruclips.net/video/sdmQSOjH7XE/видео.html), but I love your suggestion of explicitly stating the pattern of QRS conduction, (4:1, 3:1, 2:1, etc) along with stating if the rhythm is regular or not. I guess this video was meant to be super brief covering the more classic findings in each arrhythmia, which is a question I definitely get more often versus questions regarding atrial flutter with variable blocks. Finally, your last point on course afib is golden!

    Atrial flutter is usually with an atrial rate of close to 300, and conduction of a division of 300. Rate of 300=1:1 conduction, rate of 150= 2:1 conduction, rate of 100= 3;1 conduction, rate of 75=4:1 conduction, etc. Atrial rate is usually quite regular, but there may be varying ventricular conduction, either as a function of AV node, or a pacemaker slipping in a beat if ventricular response is too slow for pacer limits. Slowing the paper speed may allow flutter waves to be seen that are obscured by normal speed.

    Is the atrial flutter always regular?
    Thank You for this amazing and professional video!!

    • @medzcool
      @medzcool  3 года назад +2

      No! It depends on the degree of AV block and if it is fixed or variable. For example, aflutter with a fixed 2:1 AV block will appear regular. But if there is variability the types of blocks, as in the case of aflutter with a 2:1 to 4:1 block, then the R-R intervals will appear irregular, because the amount of p-waves between each QRS complex will vary over time.
