Don't worry, Notch has a brilliant idea: IMPOSSIBLE MODE! As soon as you enter a world in impossible mode, chaos will take over your game. Not even the best of the best can survive here. There is a reason they call it 'IMPOSSIBLE' mode.

Mobs have 50% longer notice range and attack range
15 EnderDragons in the End
Withers are common in the nether and has a 50% spawning in the over world
Ender Dragon egg can hatch and attack the player
every stone mined activates a silver fish egg
Tools lasting goes down 60%
Once you die, the world is deleted.
If multiplayer, then you can never rejoin the server.

It is always night

No mobs and monsters burn
Every zombie be a giant with full diamond armor with all highest enchantments possible and will have strength 10 with a diamond sword with all highest enchantments possible and will have speed 255
Durability on tools, weapons and armor is 5x (500%) lower for the player
Monsters have a 10x (1000%) higher attack boost and damage to the player boost
The surface is stone
Blazes will shoot out a lot of fire and lava
Water is only near the world border
Monsters have a 600% (6x) boost to their spawn rate and chance
Skeletons wear the same equipment as the zombies like the zombies in impossible mode and have speed 255
There are 30 ender dragon spawners in the end and no matter what the Ender dragons will kill you in one hit also they will spawn slimes
Slimes have a big explosion no matter what the size

There could be an extreme mode where thirst is added along side hunger. Saturation could be lower not to mention no health regen. Mobs give greater damage and given more health, or when you hit any mob, there's an increased chance that more mobs will spawn, or higher mob spawn rate, or new types of mobs could be added just for this game mode, like naturally spawning mini bosses, like giant mobs, or just really hard mobs with a ton of health.
Smaller inventory space maybe and certain ores like coal, iron and diamond could have smaller spawn sizes and spawn less frequent, so diamonds would be twice as hard to find. Even non hostile mobs could have a smaller chance of spawning.
