This is kind of sad in a way. It seems like Titus doesn't want to return to the Ultramarines. Don't misunderstand me. I'm well aware that he's PROUD of his heritage as an Ultramarine and resented becoming a Black Shield. But after his betrayal at the hands of Leandros (Whom he trusted and tried to mentor) and probably feeling abandoned/forgotten for 200 years, Titus didn't seem to keen on returning. He had found brothers in the Death Watch that he clearly bonded with and he seemed content to serve the Emperor and the Imperium in the Death Watch. Why do I think that?
He wakes up and see's an Ultramarine Chaplain. After finding out his mentor and former Chapter Master, Lord Calgar, isn't here, he immediately wants to leave. "I must get back to the Watch Fortress." No desire to reconnect with a fellow Ultramarine or speak with the Chaplain for enlightment or encouragement or any kind of grim comfort. He just wants to get away as soon as possible.
The Chaplain says "You've been reassigned." Titus is resistant as he says, "My PLACE is with the Death Watch." He sounds firm in this. He doesn't want to be anywhere else. While he had no immediate way to know he would return to the Ultramarines, I'm sure the Omega symbol on the pauldron of the Chaplain was a fair indication that he knew he would most likely be heading back to the Ultramarines. But he said his place is with the Death Watch. He doesn't want to go home anymore.
Chaplain informs him that he's basically done repentance and has been found not guilty of heresy by the Inquistion. A fuckin' miracle. Complimented by a CHAPLAIN on his service in the Death Watch. ...He looks down, almost bitterly. He doesn't take the compliment with pride or determination but what looks like resentment.
Chaplain has to basically convince him to return, stating absolution can only come if he faces the judgement of his brothers and he'll be fully redeemed if he comes back to the Ultramarines and proves himself. You normally don't have to convince someone to return, if they want to return.
Upon the words, "My pledge...is eternal service." his tone and face is bitter. This is not a proud moment for him or something he seems particularly eager for. A proud Ultramarine, a proud son of Guilliman... but with a broken heart and reluctance to wear his colors again.
I HAVE to believe there must have been some part of Leandros, deep down inside, that felt like a real asshole. The fact that he had to convince Titus to return makes me think there must be SOME part of him that feels that maybe, just maybe, he fucked up and damaged the soul of a great Ultramarine on a whim.
Kind of sad, to me.

He was in the Deathwatch, not in a cave. He probably took part in the primarch's push to relieve Bhaal from the Hive Fleet's assault.

Lore answer: Few things in the Imperium are "new," likely even though this is new armor for Titus it may have seen previous use with another Primaris Marine.
