Douma got outsmarted by the hashira with the highest iq. Also keep in mind that Akaza,Douma and Kokushibo all lost to plot. Akaza and Kokushibo should’ve regenerated their heads and massacred the hashira. And in reality Douma beats both Kanao and Inosuke then overcomes the poison
