Damages are simulated mechanically, though. Engine malfunctions, losing gears, loose traction, etc. If the damage is high enough, the truck won't start.

There's a couple of routes that are ehm, notorious for not only amounts of traffic, but also the high amount of people who can't or don't want to drive normally...Calais-Duisburg is a well-known one and I have heard some people who actively avoid that route for that particular reason.
