  • What is tuberculosis? Just like the rest of these diseases, it's caused by a bacterium, specifically Mycobacterium tuberculosis. This one is tricky because there are strains that are becoming increasingly resistant to antibiotics, so let's learn what we can about these fellas, so that we can be on our toes!

    I'm studying biochemistry and actually did my bachelor thesis about isoniazid resistance in tuberculosis. It might actually get published soon! :D

    • @bsadewitz
      @bsadewitz 4 года назад +3

      That's awesome! I am familiar with isoniazid as a serendipitous (as all the pathbreaking discoveries were until recently) discovery in psychopharmacology. IIRC, it was abandoned in favor of iproniazid, which was, in turn, abandoned in favor of isocarboxazid. Unfortunately, I only ever took one chemistry course in high school, so I don't understand much about the physicochemical aspects of all of this. Have you ever read a detailed account of the discovery of its antidepressant efficacy? It's an interesting story. I was wondering if you could briefly explain what it is about these drugs antibiotic effect especially effective for Tuberculosis, as I have never understood it. Hydrazine drugs are interesting to me because they have so many different effects. You can use phenelzine for many different conditions; it is also an inhibitor of SSAO, and its major metabolite, phenethylhydrazine, is a potent inhibitor of GABA-T and could be used as a GABAnergic drug in its own right. It probably should be, actually, as it is so much less toxic than vigabatrin. But I digress. Cheers!

      @@bsadewitz isoniazid is able to block An enzyme that is necessary for mycolic acids synthesis. These are special long fatty acids used in the membrane of these bacteria. First isoniazid needs to get activated by KatG (a Sort of catalase that oxidizes it) and then it'll go on and inhibit InhA which plays a role in the mycolic acid synthesis

      Sir how to get diagnosis for multidrug

    • @MyMinecrafter97
      @MyMinecrafter97 3 года назад

      @@ashiammi33351 Hi, i'm assuming you mean how this gets tested in the lab? They basically take a swab and try to grow it in a culture where multiple drugs at certain concentrations are added. There are defined thresholds set by WHO. If the sample manages to grow, it means it is multidrug resistant. They are also working on using genetic screening to predict the resistance level of a certain strain, but the culture method remains the golden standard for now.

    This video is quite accurate, good job on it. I would add, because I think its important to why TB remains such a problem even to this day, that the current drug regimin the antibiotic course is 6-9 months long AND the drugs make you nauseous. This can obviously cause major issues with compliance. It is difficult enough to get someone in a developed nation to take just a full course of 7 days of antibiotics....imagine trying to get people in poor areas or underdeveloped areas of the world to commit to 9 months of a 2-4 drug cocktail (4 drugs for 2 months, then 2 of those for 4-7 additional months) that makes them feel terrible and lethargic.

      I hope you get well soon.
