We use alias when we want to target multiple regions at a time. So all the regions, which are not primary, they are referred as alias and you refer those alias in your tf config file while defining the resources.

Not just regions but different aws accounts. I create a ACM cert on dev account and use alias to create a route 53 entry on a primary aws account.
