Dedicated for caliber, yes. Also, dedicated per gun IMO. Nothing says fun like having one can you shoot on a gun and then having to wait for it to cool off to move to the next gun. Budget considerations of course but definitely worth it.
When it's caliber specific, sometimes you can tell the difference in sound. A smaller caliber will work (using 5.56 in a 7.62 can) but you may not reap the full benefits of the suppressor that it's originally intended for.

If it’s a medical condition then I apologize for asking but the bumps seem to have a pattern that seems very unlikely to be a medical condition and seems more like an aesthetic enhancement. Again I don’t mean to be rude but am genuinely curious to know what it is.

There is absolutely a difference in report and signature reduction as well as sound and tone both among various .30 cal cans and different 556 cans when suppressing a 5.56 rifle. My guess is that you probably own a sandman s or a sico can lol As for your friend blowing up a can on accident, you wouldn’t have to worry about that if you had dedicated suppressors for specific rifles

It's definitely the larger displacement which litigates with the gasses better.

Depends on the can. In the case of the FLO cans, the 556K sounds better at the shooters ear on an AR. The 7.62 does better with them at the muzzle.
