No one seems to be answering the questions here so let me go ahead:
1. It's not actually a "LASER", it UV-light assisted bleaching.
2. The cost ranges from 400$-1500$ depending on where you get it done.
3. It's permanent provided you perform a home-bleaching maintenance procedure every 6 months at home.
4. After the procedure, you must stay away from any artificial colorings in your foods and drinks, stay away from cigarettes and any other staining substances.
5. This will not get your teeth pearly white, it will only remove the staining on the outer layer of your teeth and basically restore it to it's natural color. The results will vary from person to person depending on their natural tooth color.

its impossible to make them very white with this method. if you want really white teeth you need veneers which cost a lot

No teeth whitening laser is permanent because you continue to stain your teeth with food and drinks. If you want permanent whiter teeth then consider dental veneers. These do not stain or get darker.
