Check the department of education website for your specific province. I know the applications for KZN closed on 31 March 2021. I don't think the application forms for Gauteng are not yet.
Go to the department of education website for your specific province e.g kzneducation.gov.za You will find a tap for applications, thereafter click on the Examination Assistant application. If you still can’t find it, send me a dm on my Instagram @zee.mgazi, in the past I usually get a hold of the applications for KZN and Gauteng
For the past few years (pre-COVID) by March the applications would already be closed. I honestly don’t know when the applications are going to open this year. Also different provinces have different deadlines. They require a certified copy of your ID and matric certificate, a CV and you will need to fill in a Z83 form. The application form will list everything you need
