Some viewers have pointed out the value of providing the consult question(s) before providing the concise story, rather than afterwards. I think this is a great idea which I should have incorporated into the video. In my experience, this is most helpful when the likely consult question isn't obvious from the one line summary (i.e. a situation unlike the example with possible tamponade at the end of this video). Also, I find that compared to medical subspecialists, surgeons are particularly likely to want to know the consult question up front.
I absolutely adore this new series "how to..." videos you are making. I really love how you actually act out an example. Thank you so so much for these videos!
lol love the skit towards the end and subspec chowing down on a candy bar. on a serious note, i would be kind of annoyed if someone consults my service with a long story with details/fluff especially if a new intern only to find out the question towards the end. i would recommend the reverse. ask the question then elaborate on the story. 1) this gives the listener a frame of reference for the question as the story unfolds. because like you said, subspecs are busy and most of the time have a short attention span and get lost in the details. if they have a hook on a question, they can hone down on specific details of the story as you tell it. think of it as a "chief complaint followed by HPI". the chief complaint gives you the reason for admission/seek medical attention then HPI expounds it. 2) from the question they can deduce urgency of consult which might not be apparent to novice intern. overall, love your work Eric!
Thanks for the comment, and it's a good point. I think the importance/helpfulness of asking the question upfront is dependent on how obvious the question will be. For example, in this case of a person with suspected tamponade, the consultant will be able to predict pretty accurately what you are going to ask about. But if the question was less obvious, especially if it's not clear from the one line summary why you would even be calling this consulting service at all, then putting the question at the front is a great idea.
Send the MRN and question in the original page. That allows the consultant to look up the patient, gather the needed data from the chart and call your back with an answer. Also I agree that you should start the conversation with the consult questions so the consultant knows what to listen for and can identify if they are even the right person to be talking to. Also identify your role on the team when you call.

I think I've been biased because my hospital has a relatively short limit on the number of characters a page can be, and there usually is not enough space to include the patient's name, MR#, brief summary, your name, call back number, and the question(s) on top of that. But if you can fit it in, then definitely do so!
