Initial pain in acute appendicitis is due to inflammation of appendix. This is carried by t9 t10 fibers. T9 t10 also represent para umbilical region. So there's referred pain in para umbilical region. Later on as the inflammation increase, parietal peritoeum gets inflammed and localised pain in right illiac fossa occur
