    I think LVOT and aortic annulus diameters are measured in end diastole from inner edge to inner edge while sinus of valsalva and STJ are measured in mid systole leading edge to leading edg3

      Hello! The guidelines are a bit different in every country, so always check your guidelines to perform the correct measurements... 🫶🏻🥰🫀

    Hello Echo lady, on 2.02, you said sinus of valsalva or aortic root. Aren't they different? why one shall measure aortic root and SoV at the same point? I thought you do three measurements, one at root, 2nd at SoV and 3rd at STJ, the forth one is PAA

      Normally the sinus of valsalva if often referred as the aortic root... So it's the same. Most of the machines have the aortic root option but is not called sinus of valsalva. So you can measure the annulus, then the sinus of valsalva (also named aortic root), STJ and ascending aorta.

    Do u measure aortic root in diastole or systole?

      It shouldn't make any difference so just measure in the part of the cycle where you can properly see the whole "bulb"

      I was taught diastole, because you want the Aorta relaxed when getting your measurements.

    Hello, Echo Lady. If you measure the root from inner wall to inner wall, what'd be the maximum normal diameter? Thank you!

      22-34mm for females
      24-39mm for males
      Also you can index this by height according to the latest BSE guidelines. However Check the guidelines used in your country.

    When I measure the LVOT, I measure the aortic annulus from hinged point to hinged point. Am I correct?

      Some guidelines suggest to measure the LVOT at the annulus and some others up to 1cm from the annulus so depends on the guidelines you use.

    Still no numbers here to suggest a normal in comparison to aneurysm? Would you say that an MRI is needed to make the assessment for surgery?

      They typically do surgery when the ascending aorta is greater than 45 mm. 37 mm or greater is considered dilated and should be monitored. The ascending aorta can easily be measured on TTE.

      @@jhonen32629 My aorta 4.6 and subsequently my research shows that the need for surgery is 4.4 yet the aorta has not been observed for more than one time and it is predicted to expand by 1.0cm per year. I am a-symptomatic on the stenosis and have put off the surgery for a period of six months in order to procure another opinion. will i survive? The doctor has given me two years without surgery which I (literally) feel hard to believe.

      @@shoomapadoo1141 Oh ok. But definitely have someone keep an eye on it, which sounds like that’s what you’re doing.

      The numbers are different in every country and they must also be indexed by sex and BSA or height... So check the values on your country guidelines

    What is a normal diameter for the root? They said was mild dialated and I seen on my chart that is 3.55 cm

      Depends on your height or BSA... aortic diameters need to be indexed...

      The aortic diameter should be measured everytime... Not only in cases of AR, but sometimes aortic dilatation it's the reason of the AR

    Aortic root should be measured outer edge of the anterior wall to inner edge of posterior wall. Also the LVOT should be measured in systole.

      Thank you for your observation but this measurements are different in every country and even cardiac labs. I work under the British Society of echocardiography and they recommend inner edge to inner edge... Please check your guidelines.

      @@davidflerlage2463 The purpose for this is accuracy in measurements, however some studies found that there isn't that much discrepancies between both methods so I just guess is a Society preference and also a way of making everyone to do the same all the time... To standardise the echo practice.

      She is following BSE. We and world over follow ASE. as you stated ASE says, leading edge to leading edge @ end diastole and LVOTO in mid systole
